Rheumatic fever

Hi, my son who is now 8 got diagnosed with acute rheumatic fever and endocarditis when he was 4. Which left him with a bad heart. He since had valve repair but unfortunately he now needs a new valve. My question is why is he still getting the rheumatic fever rash on his thighs and arms. He is on a prophylaxis dose of penicillen and heart meds. The only time he has not got the rash is when he first got his dose of penicillen upped at 5 but now it's back. Is it normal ? His doctor hasn't got a answer to why. As every person we have seen as never seen a case of rheumatic fever.
