Saggy penis foreskin after swelling

Hello,
I have a small problem with extra saggy foreskin that I noticed
The story is as follows: during current lock down I did masturbate more often while lying on my back at home. One morning I noticed my foreskin got swollen. It looked quite big but after 2-3 days it went away BUT I started masturbating again too quickly and it came back very short. So another 2-3 days of relax, I did icing first but I noticed it didn't help and heat was better actually.
After swelling resolved I noticed my skin there is more saggy then before.
Of course I did go to urologist but he said it's very elastic there and might never come back to normal state. However what is different, and you will see it on the pictures, is that where frenulum turns into raphe it seems that it creates sort of wide canal (I am pretty sure I didn't have it before) and I can feel a small dot there....
So I am wondering whether an urologist would help me here or I am better off going to dermatologist? It looks like some sort of cyst but honestly no idea....
I included pictures below with description
#1 - raphe canal visible with clear indication where it starts
#2 - i pulled the skin showing it's basically pure skin that might be slightly swollen but otherwise looks saggy
#3 - raphe as you can see and how it got wider
#4 - penis seen from the front, no sign of swelling or so, just small wrinkles
#5 - raphe again, now going from the top to the bottom and you can see wrinkles all over... either left after swelling o still caused by this raphe-thing
#6 - tried to capture where frenulum turns into raphe and you can see a small dot there... very small but I can feel it when I touch
Going to see dermatologist soon but looking forward to any advice 😃 maybe you have had this before or I don't know....

Hello Wee__Dugie,
Apologies, please find the pictures attached
I did add 7 pictures, the last being after a bit of cold compression - you can see huge skin folds and I think it's a bit more thicker. Assuming this extra skin would be gone, it would look normal.
I have no problems with erection , urinating or anything like that. Just I can feel after entire day of walking that gravity is pulling the skin down and it hangs from there. I didn't have it before therefore I am looking for some sort of relief.
Also when I put this skin down it's looks very normal...

Sorry I did not reply to your 2nd posting with the photos as I did not seem to receive an alert for this.
Adhari has much more in-depth knowledge that me on these issues and is the best source of detail and advice on this.
Let me know, but it would be best if you update your Forum Post here after you have seen a Dermatologist for further comment

hello Wee_Dugie
so i went to the dermatologist and he said its balanitis. Right now it doesn't look bad but soon i might experience fibrosis on the skin and thickening which in long term might lead to problems with retracting foreskin.
his recommendation was to undergo circumcision as right now it's "clean" with no infection and would be easy to operate and heal.
i have primary lymphedema in my leg (and in the beginning i thought it was the main reason but it wasn't typical edema ) so i need to consult the doctor before i do anything
the dermatologist said also that he might give me some type of cream stereoid to apply but this might not only not help but actually cause some problems too. he did some steroid injections in the past but he said it was painful and didn't work the way he expected
hope this post will help someone one day 😃
thanks for the initial discussion and advice
Martin

I certainly would not know what conclusions to draw from what it is you have going on - can you state in very clear simple terms what you believe are the issues - with your penis only?

Anyone with some recommendations or anything?
I'm a bit afraid it's an uncommon condition and might take time before it's correctly diagnosed
thanks in advance!

Hi martin,
I wouldn't be surprised if the primary lymphoedema in your legs is in some way also affecting your genitals. There are different types of primary lymphoedema (Milroy disease) and in the commonest type (lymphoedema praecox, onset 1-35 years) the face, limbs and genitals may be affected.
You might want to discuss your genital problem with the dermatologist you consulted for the lymphoedema.

Hi martin,
I don't think I can add much to the discussion. There are medical conditions that can cause genital skin to swell, but these would be quite serious (and also uncommon) conditions of genital elephantiasis from filariasis (a tropical infection), TB, genital cancers, nephrotic syndrome and STDs like donovanosis and lymphogranuloma venereum. I don't think any of these are likely to be the cause of your problem and seeing a dermatologist would be sensible.

FYI- there is also a Lymphoedema forum hosted on this site. It might be worth your while re-posting some of your concerns there.
