Sciatica pain after THR

Hi all! I've had a bilateral THR as well as a revision on my right all within 3 months of one another. I am 4 weeks post op today on my last surgery. I have been having some severe pain across my lower back and down my buttock at times pain has been so debilitating I've been in bed most of the time. It started a few days before my 2 week post op appointment. I feel my surgeon blew me off by giving me more pain pills and telling me that time will make it better. After going to my PCP he scheduled a MRI and told me it could be sciatica so he pave me a medpack of steroids and I have to say I am not even a full day in with the first dose and I am already feeling so much better! Has anyone had this problem before? Just curious. I don't want to have back issues after all I've been through with my hips. I suppose when it rains it pours!

Hi jmonroe! So did you have a hip revision on same hip? And if yes what happened? I had rhr and a revision on same hip because of dislocation within 3 weeks. Not fun at all!! I didn't experience the pain you are having until after I started to weight bear. I had to go 6 weeks non weight bearing as well as no hip flexion past 80 degrees and no hip exercises. I had to battle tendonitis, bursitis, muscle weakness and now nerve pain once I started to weight bear again. I'm 7 months in. Am taking gabapentin for nerve pain. Just got through with 6 month of physical therapy. Still having joint stiffness and I limp. It's been a slow and long recovery. I take, when I need it, 1/2 a pill of tramadol. Sometimes I need the whole pill. I am getting better but it's going to be a slow painful recovery. Wishing you all the best for a speedy recovery and healing. Hang in there. Hugs 🤗 LD

Hi Linda. My first surgery on my right was 11/2 with a dislocation two weeks post op just getting off the couch. My revision was on 11/18 then I had my left hip done on 2/15. I was just shy of two weeks when I started having the pain. The pain is on my right side where I had the revision. I was scared something went wrong because I could not do any weight bearing on that leg but I was confused because the leg that I just had surgery on was feeling great. My surgeon told me it was due to being off balance for so long and my X-rays looked great. Well I am taking a round of steroids and today was my first day... it already is feeling much better. I haven't received my appointment for my MRI yet. I hope it will be something simple.

Glad you're feeling better. It's tough being in pain and having the fear of something wrong after a dislocation. That's so crazy how that happened to you!! I'm not to sure how i dislocated mine. I was told the sofa was not a good place to sit at. I had gone to rehab for 2 weeks and before i came home they brought me home to look at how things would be set up at home since I was in a wheelchair and hopping on one leg with walker. They looked at my sofa & recliner and said those would not be places i could sit in. My wheelchair became my to go chair and still is mostly outside on porch. I have my husband put it out there before he leaves for work. So did they have to redo the whole hip? I have a lot of nerve pain. Just touching my thigh on the side is so sensitive. If I could get over that I'd be good!! Well let us know how you are doing and how your appointment goes. Hugs 🤗 LD

Mine happened on my couch as well. I was getting up from a sitting position and I heard the pop and looked down and my leg was just dangling there. My surgeon told me it was like the perfect storm. The stem settled too far into my femur giving the head too much room and it just came out. He redid the whole hip. I was in surgery from 5pm until almost 10:30pm. He said the smaller stem kept dislocating when he put it back in soooo I got the biggest stem they make in both legs, he assured me that it would not happen again. But really once it happens to you you're scared to death of dislocation all the time! I am so ready to back to work and back to living on my own. Right now I'm staying with my parents, I'm only 38... but man I don't feel it!!!

Wow hearing a pop!!! I never heard anything. I did notice that my hip was higher and when I laid in bed my leg was shorter my foot was touching my ankle of the other leg. I was like that for a week or so. I was still walking and doing exercises but it was a little difficult and a little painful 😖. My dr tried to pop back in but couldn't so he cut me open again to find cup and stem were loose and the ball of hip was caught up on back side of cup. And I get scared of dislocating it again. Currently I can't get to 90 degrees hip flexion. Which is all dr allows me. I'm maybe at 83/85. Almost there but it's been slow. Do you have hip precautions? I've heard that one you dislocate a hip and they pop it back you are at higher risk of another dislocation. So are you able to move hip good?

Oh my goodness, you were like that for a week and was able to move? I was screaming, I couldn't make it off the couch. They came and took me in the ambulance. I laid like that for 24 hours and I was on morphine every two hours. But I have no precautions but they went in via the anterior approach. And other than the back pain I'm doing rather well. I still feel like my legs are different but I suppose they would with the biggest rods in there. I am so sorry your recovery is taking longer. I hope my MRI doesn't show anything else. I work in surgery and I'm so ready to go back! I'm always here if you ever need to talk. I have taken up collecting plants and bird watching. I bought a new puppy and I do massive amounts of online shopping. I better get well before I go broke!!!!

Lol 😂. I just bought a convection oven on QVC!! Wow that's something on the level of pain you were in!! I remember reading and hearing that you would be in level 10 pain if you dislocate your hip!! So when I went in for my checkup and find out I dislocated it it was a surprise to me and dr. He was amazed at how I was not in severe pain!! Well I hope all goes well for you. Thanks for the offer!! I get sad a lot cause of this pain. I'm still limping too. I had dysplasia of the right hip. You? Do you limp? Why did you have hr surgery?

Yea I was in surgery too for 5 to 6 hrs both times. My first surgery was anterior. The second had lateral. Which involved the butt muscle. Now that bothers me. Not to mention my muscles were short prior to surgery for years. Now they are stretched giving me tendonitis and bursitis. I was an 1&1/2 inches short. LD

Sadly I'm still limping but I still use my walker because of the severe pain I've been in. I hate it too. I refuse to go out beyond the grocery store and maybe a trip shopping with my mother on the weekend. My surgeries were due to osteoarthritis and hip dysplasia. He said it was the worst case he's ever seen and I was by far his youngest patient. My physical therapist discharged me yesterday because he said there wasn't much he could do with me. Really I think he was just lazy. I can think of all kinds of things to do but he said until I have my MRI i need to take it easy. Soooo I get bored so easy and at times I get down too. There is just so much I want and need to be doing. I have a 7 year old that needs attention.

Sofas are dangerous things - it was getting off the sofa that caused my hip to crumble pre-op!
I was private for my replacement, and I thought the physio was useless too. I know there are some great ones out there in the NHS but it seems there are a lot that aren't too. If you can spare a bit of cash, I'd really recommend a few sessions privately. I've had a private physio for nearly two years now, and she is my lifeline to a sensible world! Just because your are waiting for something doesn't mean there aren't things you can do to help yourself.
The other thing that is useful if you can source it is any form of hydro therapy. I'm lucky - we happen to have a private pool not far from where I live. NHS pools are now few and far between thanks to cuts, and massively over subscribed. But when I was looking for somewhere, I did notice that quite a few hotels locally had pools where they had private exercise classes that were very reasonably priced. The non weight bearing quality of water may help.

Wow you have been thru a lot.you are so brave and inspiring it's a tough surgery and so close together. I understand the fear of dislocation. It's really the reason for restrictions. Sometimes they don't always help. I can't imagine how the doc could put in a hip that would dislocate that easily.
I have a 18 year old THR that needs a new cup. I don't want to be bending over in my garden picking a flower and that hip pops out of the cup. So go in every 6 months for X-ray.YIKES, so after the revision on my other hip heals from Jan will have surgery replace the cup,in the other hip.
I have had revision Jan 2017, in surgery for 5 hours. They have to take all the explants out and add new ball joints and larger femur implant since the smaller size was removed. They broke the greater trochanter when removing the explant in the femur. then a cable was wrapped around the femur greater trochanter to hold the new implant and fracture in femur together. So now 3 months on walker is expected since fracture included in the revision.
I am in no,pain doin well and in a couple,of months looking forward to walking thru the
grocery store without a cane or walker. I have been thru 2 weeks PT in rehab 3 days after revision 3 times a day. Now I'm home and lazy just resting. Some days have energy others not so much. It's slowly coming back.

Yes, the same thing happened to me. I had previously (2 years prior) suffered badly with sciatica which resulted in surgery to correct it, so it terrified me that my back might be messed up again. My orthopedic surgeon put me on steriods for about a week, ice packs and continued the pain meds. Thankfully within 2-3 days the sciatic pain diminished.
There is a great deal of swelling of tissue and muscles after a FHR. The swelling can press against other nerves, which in my case caused the discomfort from the sciatic nerve. The good news is that after the steriod treatment, it didn't come back.
With time you should be fine. Best of luck on your recovery
