sertraline dosages

I have been on Sertraline for 3 years. I have generalized anxiety with OCD and panic attacks. I was completely symptom free while on Paxil. After 25 years I started to have anxiety attacks so my GP changed me to Escitalopram. it managed my symptoms at about 80%. After two years I needed to change again. Now my Setraline is not working as well and my GP increased my dosage to 150. There has been an improvement but still have parts of each day that are kind of rough. Has anyone ever taken 200 mg? Also is my age why the medications don't work as well? I am 73. I have been diagnosed with Polymyalgia Rhuematica and I wonder if that has an affect on my mental health?
