Sharp Pain near bladder and bleeding post urethrotomy (1 week)

Hi there.

I have urethral stricture problem in the bulbar region of urethra. i had my first oiu urethrotomy in October 2018. Stricture length was 1 mm.

Unfortunately had recurrence of urethral stricture (4mm) and had to go through urethrotomy again on July 23 2019. This time doctor put Foley catheter fr 20 for a week which was removed yesterday.

After the removal, my flow has significantly improved. However this time i have noticed that during urination, i have sharp pain somewhere near bladder (3 inches below navel) which lasts for ~5 seconds. Also i have noticed slightly more bleeding (8-9 drops) and clots after urination. My current medication is antibiotic Septrin DS 100 (1 tab/day for month).

Is above common to expect after a week of urethrotomy? How long it would take to bleeding subside? Is this due to catheter? Feedback would be greatly appreciated.

Thanks for your help!

Ismail

PS: I read one article on nihs regarding similar matter and I am little worried.

Generally bleeding shall subside in how many days after catheter removal? Right now blood comes immediately after urine is passed and is dark red.

      thanks for help!

it should bot be painful and the bleeding should subside in like 5-7 days it should become lighter

pain shouldn't be there or it should be very minimal

After my first urethrotomy in last year 2018, i had zero pain. Also there was hardly any bleeding after a week of procedure.

The second episode of urethrotomy has been different for both bleeding and pain. It is almost 10 days now. I hope it ill get better from here.

I will keep posted regarding subject. Many thanks again for all the help and advices.

maybe an anti-inflammatory will help settle things down. i always found it worked after my urethral dilatations.

    all the best

      Thanks a lot for reply.

I will take this up tmr with my urologist. Currently it is pretty painful.

you should go to a different urologist. your doctor may have made your urethra worse like mine did after my second urethrotomy 3 and a half years ago. hes not going to admit that to you. my pain has been constant while I wait for new strictures to obstruct my penis now too. I too originally only had a short bulgar stricture. instrumentation in the urethra can be devastating if your doctor doesnt know what hes doing or doesnt

      Hi Gomer44

Thanks for your reply. sorry for getting back late. I have visited two urologists for their opinion. For now I am continuing with same urologist but will keep a close check.

It is exactly a month now. Pain is considerably less. I took antibiotic Cefrofloaxin for 2 weeks.

There has been a some drop in the urine flow. I will have dilation procedure started from today. Hope this goes well. Wish me luck and health please. 😃

Thanks again for your time. Hope are doing better now.

      Sorry for late reply.

It has been almost 3 weeks now. My urologist used catherter of 20fr for the dilation. Left it there for 10 minutes.

Results have been pretty good so far. Actually i didnt feel this better after my urethrotomy. What a relief is to pass urine normally without any pain! My uriflowmetry result after dilation was Qmax=47ml/s which was very good as per doctor.

Hoping for good and keeping fingers crossed for future.

    Hello ismail,

hope you are doing well. could you confirm if your strictures length are in cm's but not mm's. Usually strictures will be measured in cm's as far as i know. anyways, in my opinion after your first urethrotomy you have mentioned that the length of your stricture is increased. At this point you should have directly gone for urethroplasty. Since your stricture is in bulbar region, a BMG urethroplasty is suitable. why dont you take a second opinion because performing urethrotomy for longer strictures is not recommended and for longer strictures (>1cm) its better to go for urethroplasty. i had a stricture of 3cms and urethrotomy was failed. recently i had urethroplasty, however i can only confirm the success rate after an year (9 more months to go). But with urethrotomy for longer strictures, there is a possibility to make the stricyure more worse.

      Hi Victor

Hope you are doing well. I went through one more urethrotomy in June 2020 which is my third. Actually i had my urethroplasty planned in my native country this time but due to covid restrictions I couldnt travel back home. Tried urethroplasty here in my expat country but couldnt get it (covid reasons).

My current stricture length is 3cm. i will now go for urethroplasty by end 2020. How is your urethroplasty going? Hope to hear from you soon. Prayers for health and safety. Take care.
