Sharp rectal pain on both lateral sides. Fissure or something worse??

hello all, i have been suffering from rectal issues for 3 months. I was diagnosed with a fissure before but i was told it healed. i still had rectal irritation and pain, was diagnosed internal hemorrhoid and did 3 banding sessions with the o'regan system.

now i am still having sharp rectal pain, and it feels like it come from my lateral sides.... today i was applying some anusol and im pretty sure i felt a sore seam on my left side....

i know fissure dont usually appear there, so im worried what it might be... i do feel more pressure at that location when having BMs...
