Should I go to the ER?

I have had a constant headache for five years now. It hurts my whole head but the left side more. It feels like pressure on the back of my left eye too. Last MRI scan was more than 3 years ago and I'm on no medications for it. I tried almost every headache medicine (NSAIDS and prescription) and nothing worked. Recently, my headache pain has been getting worst. And I'm starting to have alarming symptoms. I keep twitching, vomiting, confusion, difficulty to speak sometimes, and hard to understand people. These symptoms and headache severity formed a couple days ago. Do any of you know what could possibly be going on? Do you think I should go to the ER or something ?

    Also forgot to mention that is hurts extremely bad but not to the point of crying. I would say a seven if I had to rate it
