Should I wait for Zeiss LARA in the US or get Symfony?

LARA promises 0.5D more range and lesser night time issues with the SMP.  Anyone has any ideas on this.

I did extensive research and had an ATlara lens implanted into my right eye yesterday at an eye surgical center in the Country of Panama. I developed a traumatic cataract in my right eye 2 years ago after a skull fracture. I was using Bio-Infinity multifocal contacts with great success for the last 3 years until the cloudiness of the cataract diminished the vision in my right eye to the point of me needing to go for the lens replacement. I would recommend the Zeiss Atlara lens for anyone considering this type of surgery. The testimonials and reviews of this lens from both doctors and patients blow speak for themselves. I just had my 24 post OP follow up with my Dr. and I am already 20/20 at all distances in my right eye. I will continue to use the Bio Infinity contacts in my left eye but am already considering replacing that lens also after the results I am already experiencing. Some slight crown like star bursts last night looking at bright lights but again that was only 7 hours after surgery. the doctor said it will take weeks for my pupils to fully adjust where it can take advantage of all the areas of my new lens. I highly recommend this lens and also the Dr. I used. One could fly to Panama City and get the entire process done in 10 days including preoperative measurements and the one week follow up post op exam. The cost was reasonable in my opinion and I paid for it out of pocket.

What did you end up doing? And are you happy with the result?

I had a lens exchange 5 months ago in one eye (Zeiss' AT LISA) . I see a large circle in the middle of my field of vision when I walk by a light, from the side (like street lights) or from above (ceiling lights). The ring gets smaller or larger depending on where I'm looking. The reflection comes from a light source, but it is not a small ring around the light source itself (concentric circle) – I can live with those.

It is worst at night, but also present at day time.

I am sure that it is edge glares because it helps to take pupil constrictor eye drops. But they have so many side effects that I can't take them.

I am planning a replacement for another lens (J&J Eyhance), but I am nervous it wont have an affect on the reflections, because I think it is the edge of the lense, not the rings in the trifocal iol, that causes the large halos.

i got symfony and suffered with extreme edge glare for the last 5 years. the edge glare is worse with all JnJ lenses even the monofocal one.

i got panoptix in the other eye in May and the edge glare is minimal. so i think the panoptix suits me better.

Thank you so much for your reply!! It really makes me reconsider whether Eyhance is the right choice. I am sorry to hear that you are still fitting with this.

According to J&J their IOLs has a frosted edge to minimize edge glare. Do you know if that was the case for Symphony when you got the surgery five years ago?

My other candidate is Alcon Clareon monofocal.

J&J's and Alcon's IOLs both have an optic diameter of 6 mm, but as I understand it, J&J only uses 5.1 mm which makes a dead zone (compared to Alcon's) in dim light for people with large pupils. Maybe that also increases the risk of edge glare for J&J's IOLs. Could be interesting to know.

frosted edge doesnt do anything for me. yes symfony had it 5 years ago.

i have had good experience with clareon. i see edge with clareon too but the edge is very small and constricted near the light source. the symfony edge circle is size of a building.

yes that dead zone is not a good idea for those with large pupils like myself. dr did point that out to me. for panoptix he said the outer rim is powered for distance correction in addition to the central zone.

It sounds terrible. Where in your field of vision do you see the edge glare? And what shape does the glare take?

I see a circle (some times just a part of a circle) in the middle of my filed of vision when I walk by a light source. The circle changes size depending on how far I am from the light source (maybe the light jumps from circle to circle in my multifocal iol, or maybe it is just because the angle of incidence of the light changes when I move. I hope it's the former because then switching to a monofocal lens will help).

I have seen three ophthalmologists/surgeons and no one has heard of it or has any idea what is going on.

And may I ask why you went with the panoptix instead of the monofocal? (I am terrified of having PD problems with another multifocal)

Also, did you consider the 7 mm Aspira-aXA?

arc of light will form nasal upward - 11 o clock to 4 o clock in the left eye. it becomes a complete circle in more darkness.

i did consider aspira and think it will solve my problem. i went with another multifocal when i found people with eyhance and tecnis monofocal experiencing same symptoms.

thanks for the reply. really appreciate it. Mine is also from 11 to 4 (but in RE) when it takes the shape of an arc, for the most part, however, it is a complete circle.

i develop nasal upward arc 8 o clock to 1 o clock in the panoptix eye too but it happens infrequently and the arc size is very small when compared to tecnis.

Have you ever had your PCO removed with YAG laser? I wonder if it will help in relation to the halos/large arc of light.

    • Edited

i have not. i believe it will change the immediate full circle edge to flares of arcs. that's how it started 6 days after surgery.

i am 3 months out panoptix and i can clearly notice the difference. example the ipad has a sizeable glow halo around it with symfony. panoptix is clear. no halo around the ipad.

I am glad the panoptix is working for you. At least.

      Stupid annoying halos.

my complaint with panoptix is that point light sources outside at night will have a comet like tail to lower left. it reduces when correcting astigmatism with glasses but doesnt go away completely. otherwise the panoptix is gentler for me.
