Should I worry?

Hi. Had a digital exam this morning and Dr says I have an enlarged prostate. He said it was common in older men, I'm 72, but wants a PSA test done anyway. It's always been recently described as large but I have no trouble passing urine etc. I have had a major scare with bowel cancer and this has left me anxious about anything which has a cancer possibility. The 5 year survival for the bowel cancer is looking ok, I'm over four years now. He said he couldn't detect any lumps and any cancer takes years to develop. So can I stop worrying a little? Any advice from anyone who knows about this topic will be appreciated. Thanks.

    I agree with your physician who suggested route of A.S. and that it is unlikely to get worse.

    You have a 3+4 which is the low end of Cancer.

    3+3 in fact is not thought of as cancer.

    You have nothing to rush into.

    I see you may consider radiation. Note that there are 2 forms offered: high dose over a short period eg 3 or 4 sessions versus low dose offered over 5 or so weeks.

    Good luck.

    Hear what the oncologist suggests.

    Then think it over.

    You have no need to hurry.

      Thanks Barney. That is reassuring. I have to ring the hospital on Thursday to see if there is a time available. They have had a talk about it so hopefully it won't be too much of a wait.

      Ideally you will be seen by a urologist who has high volume of patients. The more patients he sees, the more experienced he is and the better the decision as to whether to treat or not.

      Don't make the mistake many make and go to the Dr nearest them. Just because he is near is not a reason to see someone.

      As far as I know there are only two specialists where I live and they also see patients in another city. He is going to ring me to discuss the next step in the next few days. We shall see!

    John, I had a similar diagnosis over 2 years ago. My PSA was 5.7, biopsy showed 2 out of 12 core samples with cancer cells. It was adenocarcinoma which the urologist said was a slow growing non aggressive type. My Gleason score was 6.5. I had time to think about options including A.S. I met with an oncologist that works along with my urologist. He talked to me about treatment options. He drew diagrams and charts showing me the pros and cons of each treatment. He also talked to me about Brachytherapy along with other options. Radiation treatments that would be short daily treatments for 32 visits. Another radiation treatment was called the Cyberknife. That was 5 high dose treatments at 45 minutes each. There is some preparation for each visit too for the radiation treatments. The Brachytherapy he told me was a one time treatment done as an outpatient. It takes about 45 minutes under anesthesia. They implant low dose radiation seeds into the prostate. And over a period of a few months, the radiation does it's job and the radiation is gone. The positive side to this treatment is that the radiation is low dose, so less chance of burn surrounding tissues like the colon and bladder. I liked the once and you're done option so I made the decision to have the Brachytherapy and be done with it. As with any treatment, there are side effects. And I had some with this treatment. Ibuprofen helps with the discomfort. And I took Flowmax before the procedure as preventative to keep the urine flow. The radiation causes some inflammation to the urethra. I also took Avodart for a while. After 6 months, all was pretty much back to normal. After the year mark it was like nothing ever happened. I don't take the Avodart or the Flomax any longer. .. which is great. No incontinence, no erectile dysfunction. My PSA at last check a couple of months ago was 0.14. If you can go for the A.S., that is good. But if you decide on a treatment, make sure the doctor is experienced and has performed that procedure many times successfully. Good luck to you.

      Thank you Charles! That was very useful and informative. The Brachytherapy is apparently not available where I live and possibly only privately anyway. But it does sound like a good option. I shall read your reply a few more times and get ready for my ph consultation in a few days with more knowledge. I have had a large part of my bowel removed which might make radiation a bit risky? It was an extended right hemicolectomy almost five years ago. Thanks again.

      Yes I would think the radiation would be a concern due to your colon surgery. A good question for your physician. I am in the U.S. and have medicare as my primary insurance. I also have a medicare supplement. I had no out of pocket expenses for the Brachytherapy. Hopefully your doctor can offer some good options for you... even if he decides the active surveillance is best. I had that as an option, but decided if I could fight it and be done with it, I would rather do that. I do wish you the best. I would be interested in what your doctor has to offer. - Charles

      The health care in New Zealand is tax payer funded and you can also buy private health insurance which shortens waiting times etc. I stopped private insurance as the premiums shot up as I got older. Brachytherapy isn't available in my city and is only available privately, for about $25,000 nz I think. I may be wrong. So AS is looking more and more likely. We shall soon find out I hope!
