Shoulder impingement

11 weeks ago I awoke not being able to move my left shoulder it eventually popped and was very painful but had only minimal movement. The doctor thought I had a rotator cuff injury, perhaps a tear. He told me to rest it and take anti-inflammatories for a week. There was no improvement so he gave me an injection . Very very slight improvement. I then had an x ray which after 4 weeks waiting for the results said there was nothing abnormal but I had no improvement in the mean time. Had an ultrasound scan which showed a marked enlargement of the tendon. Before the scan I had physio where she said she thought it was more to do with my neck and nerves. First lot of exercises led to complete agony and very little movement. Gave me other exercises but was then off sick and they missed me off the list for her replacement. Now got another physio who tells me the enlarged tendon is only a symptom and the cause is me tensing because I have been in so much pain. My work is threatening to dismiss me as I cannot tell them when I will be able to return so I decided to pay for a private shoulder specialist as no one has even considered what caused the lack of movement in the first place. The physio thinks I am wasting my money as she says he will operate and I will still have to have the same physio but will take even longer to heal. I am at a total loss as to bother with the shoulder specialist or to continue with the physio. All she has told me to do is practice deep breathing without tensing for the next 2 weeks. Has anyone got any advice or been in a similar position. Thank you

    I would go and see the shoulder specialist , it seems that you need to know what the diagnosis is , and get realistic expectations on recovery time . Money well spent?

      Thanks for replying. I have got an appt on 27th April. Hopefully the £200 will be well spent. He is apparently the top man so fingers crossed. It's been very frustrating as I am not the sort of person that can go in all guns blazing and I've always in the past had very food advice. I understand that shoulders are one of the most difficult parts of the body to treat as their is so much that can for wrong!

      Good luck and I hope you get the answers you want , dont be disappointed if he/she suggests more physio at first - esp if they have a shoulder physio - most will avoid surgery if they can for obvious reasons .Also you might need an MRI. I've had a supraspinatus tendinitis and impingement for nearly a year , went to see specialist who did a cortisone injection which helped for 3 weeks but having more physio still . I want to avoid surgery and I realise I have got to do the physio properly and get my fitness/ strength improved .There are no easy answers, its just so relentless!

      I ageee with Stephanie that GPs are not the best to deal with persistent shoulders unless it is their particular interest as they don't deal with enough and can't be expected to know in depth .( and I am one)

      Hi. I was sent down the physio route. Everytime it just aggravated my symptoms and was left in agony. 10 months after initial fall I was told by a physio that my shoulder hurt 'because I wasn't using it!' ' I needed to relax more ....' I disagreed. 3 weeks later the specialist noticed off my original MRI scan from 9 months previous that I had no cartilage left in my shoulder. Every time I used my shoulder the bones rubbed against each other. Hence the pain. Surgery the following day. Listen to your body and don't be fobbed off. Do your research. Ask questions. Take someone with you or write everything down. When you are in pain and on medication your head is not always in a good place to take in what is being said.

      So the problem was not the physios fault as he/ she was not to know that you had no cartilage! The fault was not reading the MRI properly

      Partly as the physio wouldn't listen when I kept saying. 'It hurts when I move it, not that it hurts because I don't use it'. Then told to relax more! Just so frustrating. Would be so much easier when there is a wound, swelling or bruising for them to see. I understand that they are all highly trained professionals but sometimes they need to stop and listen to their patients more.

      It's a Mr Dixon at the Duchy Hospital in Cornwall. I think when people say he's the top man they probably mean in Cornwall as many see Cornwall as separate to England!

      Could well be the top dog. My last two surgeons were 'top dog' but they retired mid treatment! Good luck hope it all works out for you.

      I went to see the specialist yesterday. It appears that although I have shoulder impingement the cause is my neck and nerves in the neck. The muscles in my shoulder are not working at all. He has suggested physio on my ne ck and shoulder muscles. Am hoping that will work but he has advised that it may not cure the impingement in which case I shall have to have an operation. He has also said that if physio doesn't work then I would have to see a neurosurgeon about possible neck surgery. He is sending me for a MRI scan. Although he could not give me any real timescale he said I was looking at the very minimum of 3 months. As I am a support worker for physically and learning disabled adults I think the likelihood is I will lose my job. Trying to remain positive as there are so many people a lot worse off than myself!

      Just to update you on what has happened. I went to see the specialist who in 5 mins decided it was my neck that was causing the problem. I had a MRI and the results showed I have 2 trapped nerves in my neck. I have been having physio but had to have nerve damage tablets as it caused quite a lot of pain. On looking up what they have said is wrong on the NHS website and it said gentle physio, no manipulation. My physio has been doing all manipulation and I am now concerned she has done more damage. My shoulder is giving me more pain too. I'm seeing the specialist on 30th June so decided to stop the physio til I hear what he has to say.

    Hi Alison, so sorry to hear about your pain. I totally understand and sympathise. 6 years ago I had a fall. Minor but annoying bicep pain just wouldn't go away. Gradually deteriorating. Diagnosed with Impingement.......... needless to say I was messed around. Treated for frozen shoulder, 2 bicep tenodesis and more. My best care was by a private specialist. Everything else was !!!! Pursue the private route if you can. I lost my job because of public patient waiting list. Take control of your symptoms. No one else will. Any specific questions please ask I might be able to help.       Thanks for replying. I've got an appt on 27th April. What I'm wanting to know is cause and whether the work I do is likely to cause it to happen. Treatment as the physics I've been seeing have done rotally different things and I was told if I saw another one while she was away they would probably do something different again! That makes no sense after having the scan. The other thing is time as my employers are saying that unless I can give them some sort of timeline they are able to dismiss me. Reading all the different discussions on this forum it appearsGPs are not best places to sort shoulders out so I would have thought as soon as anti-inflammatory tablets or injections don't work they should be looking at sending patients to shoulder specialists. All down to budgets I suppose. Can I ask what your treatment was and how long recovery time was. Thank you.

      Hi. I had 2 X MUA (manipulation under Anaethestic) treatment for a frozen shoulder even though I was never diagnosed with that condition but impingement. 13 cortisone injections over a few years may have led to the degeneration of the bicep tendon. Bicep Tendosis X 2 then finally last year a re location of Bicep tendon from under pec major to over the muscle. Debilitating pain that isn't visible to others but encompasses your whole life. Occurred on right arm. Can't write/type for any length of time. Now struggling with a form of tennis elbow. Too many different medics. Lack of continuity of care. Best was from BUPA specialist at Spire Bushey. Started off after a simple fall and placed arm out behind me. Niggling pain now on Oxycodone after taking Tramadol. Was only 44 at the time. I knew nothing before I started all this. Now I feel more educated than some of the doctors!

      Wow sounds as though you have really been through it. They started me on physio before they even knew what was causing me not to be able to move my arm more than about 20 degrees which caused an enormous amount of pain.n I think I will end up losing my job as I am a support worker working with physically and mentally disabled adults. Reading between the lines it appears once you have a problem with your shoulder no matter what treatment or how much you recover their will always be a weakness their. As I do not have a partner and a mortgage it is very worrying and I don't think all that added stress helps to become less tense!

      Hi Stephanie. Been to the specialist and it appears the problem is my neck and nerves. He is sending me for a MRI scan and am looking at physio. He's not sure whether it will sort the shoulder impingement out in which case I will have to have an operation. Hoping the physio will work on my neck or I will have to see a neurosurgeon with the possibility of an operation. Remaining positive  about physio but likely to lose my job as the very minimum recovery he thinks will be 3 months.

      Hi. Thank you for the update. So sorry to hear about impingement. I've been down that route along with MRI scan etc. Just hope the best for you that your Consultant is better than the Registras I was messed about by. 😉

      Now its more to do with my neck and nerves I probably won't see him again. At the mercy of the physio who has told me today that my top 6 vertebrae are at all angles and need to be put back in alignment. This has caused the nerves to be majirly irritated. Without me knowing it my body has been trying to sort itself out but in the end majing itself worse. She tells me it's going to take some time to get it all sorted! Hope you can get your problems sorted too.😊

      I wish you all the best with your physio. Please keep in touch. Would really like too ie the treatment offereded. My neck, a line from behind my ear running down my neck to over the back of shoulder is tighter than any knot you can think of. According to a Consultant Anesthestic but did not offer anything to remedy the pain. Curious what you are offered. It affects my arm as well. 😉

      I'll let you kniw how I get on. Reading all the different problems associayed with shoulders no two peiple seem to get the same treatment and recovery seems to be cery slow. I'm a support wormer for physically and learning disabled adults so I think I'm going to have to give that up. Being without a partner and having a mortgage to pay is somewhat worrying.

      I feel for you. When I started (injury treatment) I was married (20yrs) 5bed house, mortgage, a job 4 children the package. Now single parent(divorced) , social rented (moved twice) and 1 child in college still battling pain both shoulder and now damaged knee thanks to a nasty fall when I collapsed at home caused by shoulder pain awaiting delayed shoulder surgery and approaching the big 5 0 in just over 2 months. 😜

      You really have had it tough. I'm 52 and I have been looking through job sites. I used to do book jeeping and admin jobs but that was 22 years ago and the thought of doing that again does not appeal. With little movement and no clear indication of how long things will take I don't know what to do. It's lucky i'm ever the optimist so keep smiling!
