IMPRESSION:

  1. High-grade partial-thickness articular-sided tear in the anterior and mid fibers of the distal supraspinatus tendon adjacent to the humeral insertion. Mild tendinosis of the infraspinatus tendon and a superimposed low-grade partial- thickness articular-sided tear in the anterior fibers of the distal tendon adjacent to the humeral insertion. Focal linear intermediate-grade partial-thickness articular-sided tear in the superior fibers of the distal subscapularis tendon.

consistent with a high-grade partial-thickness articular-sided tear of the supraspinatus tendon which involves greater than 75% of the tendon thickness.

Biomechanical studies have demonstrated that in the presence of a partial thickness tear, the strain patterns within the remaining intact rotator cuff are impacted. This coupled with the limited potential for spontaneous healing predisposes the tissue to tear progression.

While conservative treatment is often effective in reducing pain and improving range of motion and function, persistent symptoms and disability may be present warranting surgical intervention.

Conservative treatment = therapy, possibly more injections, nsaids. This may contain the pain to a greater or lesser degree and improve function to a greater or lesser degree. But the overall message from your orthopedic specialist will probably contain somekind of prognosis for possible surgery in the future - near or immediate or sometime down the line.

My POV: better sooner than later so you can become healthier.
