Small prostate FLA for BPH any success?

Posted , 9 users are following.

Seriously considering FLA as I’ve been diagnosed with BPH.

Urodynamics look good, ultrasound looks decent, 3tMRI shows some pinching and bladder neck protrusion.

I  am sitting on a small prostate..30cc, 250ml PVR, no prostititis, get up once a night, poor flow, urgent a few times a day.

Anyone small prostate having success with FLA? Or?

I probably wouldn't even consider surgery in your situation. If you've been following this  site, you may recall there have been many men who had surgery and were very unhappy they did. I think opting for surgery without a real need for it would be a mistake.

What else todo? Bladder is being compromised, retention and trebeculation are on the rise.

I figure I’ve waited a year lurking in forums and weighing the options so I thought I’d grab a little more intel from the field of patients before I commit to a plan.

If I could cath without the the little blobs of blood I’d wait longer. Just can’t seem to get it passed the sphincter easily.

Hey buddy I would recommend caution as well. I to have a small prostate at 42. I have had PAE, and FLA. I have not really had any good results from either.I still get up to pee at least 3 times per night. Very poor stream, and urgency I never really had before. I also have partial retro ejaculation, after having the FLA. And neither of the procedures are cheap at all. Have you tried any of the meds before thinking about surgery? I think you have the same attitude I do, if something is broke lets get it fixed. Unfortunately when it comes the the damned prostate it just aint that easy.

    Good luck J

Hi joe, ya I tried following your timeline on this site and wondered how you were. I’ve done the meds flomax and rapaflo and cialis with meager results. I’ve tried self catching 5 times and 3 times drew some times blobs of blood.

talk to Dr.K and my urologist and both think I need a procedure.

I have been holding off for a year. I’m stunned that my prostate is within normal size and everything else is working. The doctors warn me of stretching the bladder out.

      Somwhat other option is there?!

Did  I read your brother also had a small prostate? what happened with him?

with a prostate that small ,I think I would consider urolift. That said I think urolift is a stop gap measure so I wouldn't consider it for my 125cc bundle of joy

Hi, I had fla with a medium size prostate with a median lobe pushing against my bladder closing off much of he flow and made the mistake of leaving it too long and although the fla was 100% successful by Dr Karamanian the bladder has to be retrained to remember how to hold urine, however despite the bladder the fla was amazingly successful and without pain either during or after fla, I am all for recommending avoiding surgery where possible but in the case of a median lobe surgery ends up being the final conclusion, I am emailing you to recommend having a chat with Dr K and can say with confidence that he is not interested in your wallet but whats best for you, I know it sounds like a load of rubbish but trust me he is not a self centered money grabbing quack, he will be happy to advise you but you will need your medical history details ready at hand all the best kind regards Brian.

Hi Brian, yes spoke with Dr. K

Did you mean retrain the bladder to empty? Or urgency?

      how big was your prostate?

super glad to hear the operation was good.

Hi, in my case I previous to fla had severe urgency and could not empty properly so forever looking for a place to pee and having to go back repeatedly and severe pain if holding on too long and even worse pain when finally able to pee! plus endless broken sleep sometimes 8 wake ups/night

After fla with Dr K much better!, immediate relief from the above relentless torture, could pee easily, no more pain if I held on and no pain to finally pee and now sweat sleeps maybe get up 1-3 times even when drinking much more fluid at night than before but noticed when I don't have much fluid to get rid of the feeling of not fully emptying which is the bladder not pushing all urine out because of being unable too all those years and now needing to be retrained by drinking more fluid and purposely holding on longer and then peeing easily with great flow but not fully emptying out yet retraining bladder to fully empty, also still have a degree of unnecessary urgency perhaps from some kind of mental fear or mental habit  but its gradually fading away as my confidence grows, so all in all Dr K saved me from an absolute horror story of being trapped in eternal peeing nightmare!

Exactly I am concerned about procrastinating.

i am on silodosin a generic form of rapaflo. It’s not great so I also add some cialis.

The urgency is tolerable. I drink a lot of fluid to help prevent infection.

The flow is not as good as the urgency though and it’s getting worse. And when I do self cath or have a bladder scan there is always around 200-250 ml as the post void residual.

you u definitely can retrain the bladder and I hear if you CIC you can help it along faster. Best of luck on that

Hey thanks, you asked size of prostate, about 55cc or approx 4cm widex6.5cm tall(enlarged median lobe, ps from what I have discerned it may be in your interests to check with Dr K about use of drug types or supplement types, remember dont push when you pee to prevent damage to kidneys with poor flow, all the best kind regards Brian.

Why does the fallacy endure that prostate size matters when it comes to obstructive symptoms ???? It DOES NOT. Period. End of story. Good golly. Men with gigantic prostates can have NO BPH symptoms. Men with small prostates can have significant symptoms. What matters is WHY. WHY. WHY there is obstruction/reduced flow. That is why you invest the time, money and effort into diagnostic workups. Now that I have that off my chest...I am a prime example of why size DOES NOT MATTER:

Pinching and protrusion may very well be a great candidate for FLA, simply upload your 3tMRI results to Dr Karamanian and see what he says. So easy, and FREE. He is honest and trustworthy and will tell you if you are a risky or poor candidate for FLA.

Best of luck from a small prostate guy who, thanks to Dr. K, went from mid-20's IPSS to currently a score of THREE. Nine months post-op I keep getting better.

      Thanks Pete!

I guess there is so much at stake that any negativity fires up the amygdala. So little data,  ergo fear of the unknown. Your contribution is a huge help to support FLA.

Ill check out out your link... what size was your prostate?

I did speak at length with Dr.K  and still think it’s my best option. If it weren’t in a foreign country and expensive I would be able to do it anytime.

Great to hear about small prostate success to help balance out the information in the forum.
