So here's where I run into a problem....

Have been using the DSNS to reduce from 13.5 to 13 of prednisone.

Did OK with the first 1/2 of the method. However, today was the 2nd day of the send 1/2 of the plan where I went from 1 day old (13.5) to 1 day new (13) and tomorrow I am supposed to take 1 day new ( 13) again.

Problem is I can feel the pain and stiffness returning to the point that it is affecting my day. I will most likely go ahead with the plan to take 13 tomorrow and see how I react.

My dilemma is if it gets worse after tomorrow's dose where should I go back to ?

If I was OK at 1 day new (13) and 2 or 3 days old (13.5) is that where I go back to and pick up again from there ?

    This happened to me too. I went back to the old dose and stayed there for about 2 months then tried again. Second attempt was successful and I have been able to continue my way down currently at 8mg. Slow and steady wins the race I feel. You need what you need and can't get any lower until your body lets you. You may even need to go up a couple of mg for a few days to settle the inflammation before dropping back to 13.5 but you'll be the judge of that. Hope it settles for you.

    You can do any of the combinations... For example go back to 13.5 for few days and then try 2x old 1x new or just simply go back to old dose until you get back to "normal" and then start taper again. DSNS is just a suggestion, not etched in stone, you can create your own taper as you need.

    Having said all that, if I were you I would just go back to old dose and try taper in few weeks again...

    I assume that you are sure the pain is coming from PMR rather than something else. If so, there is the problem of overshoot. Even on DSNS, the return of pain may not be halted by a simple return to the last "successful" dose. Often, inflammation has already built up, perhaps before it manifests as pain, and you are likely to have to go back a good few mg to control it.

    That is what I'd do and see if that holds it. If not, back to the last dose, wait a month or two (or three) and try again. Some people find it takes a few attempts, some more. It doesn't mean you won't get there - just not yet. If just marking time isn't enough, try adding a few extra mg for a few days and dropping back.
