Spasms and gurgles in bladder area

I am male, 66. Most recent psa 0.74.

I have a long history of urinary problems, hesitancy, frequency. Seen a Urologist. Most recent appointment was a telephone consultation. Since about November 2020 I have been getting what I can only call spasms, like twitches in lower abdominal area. But also gurgling feelings and sounds which at times are almost audible, like bowel when wind is there. But these feelings are definitely over lower abdomen,central just above pubic bone.

My urinary symptoms have worsened recently, with great urge to pee, and very frequently. The gurgling and twitching is continuing and mild discomfort in bladder area. No-one seems to have heard of this gurgling. Any idea what is going on?
