spinal cord stimulator

Hi i have been reffered by a physio therapist from nurological unit for a spinal cord stimulator, i have received a lot of forms to fill in which includes a section on mental health with questions like have i felt like suicide or self harm etc, as well as questions on my pain levels, the letter also stated they would be asking my GP for full medical historty, is this normal prior tom seeing the consultant? i had flexible spinal fusion 2008 which seemed to work for a while but i have been in constant pain and on strong painkillers for last 4 years. I have had facet joint injecdtions which gave some comfort for around 6 months and am waiting a further injection soon. I have been pushed from pillar to post over last few years mainly due to consultants off sick with STRES!! I am worried  answering the mental health questionaire incase i put wrong thing and i dont have opp , i am not on any stress meds or depressed. Please help as am confused why i would be sent this form or is it normal thanks julia

    I was supposed to go but never had the appointment, I just had my nervo senza put in sept 24th I would answer the questions truthfully, I'm sure you don't want to hurt yourself or any of that but I would question why they want your gp info

    That seems a little bit prying into your personal health, if it doesn't have any thing to do what you are getting I would be like NO, thats my personal health history.

      thank you for your reply i will see my GP to see why they need all that stuff when they know i have had failed back surgery i cant wait for the facet joint injection in november , it has helped the pain last time i had over a year ago. The injection is to tied me over untill i have the spinal cord stimulator apparnetly the waiting is 12 to 18 months !! god hope not as in so much pain inspite of strong painkillers.

      I was skeptical about it but my legs and back were getting so weak that I had it done, but I am going through some issue's, I'm in such pain still from the surgery sites plus the legs and back, no sleep, if you can get the nevro senza no sensations just hopefully pain free soon. I'm hanging in there. But its a bumpy ride I hope in the long it works.

    Unless you have been diagnosed with mental illness and you are on medicine for it, I would not give them any information. That will stay on your records for good. They are just protecting themselves from any law suits. They do not want you to come back later and say that the stimulator made you crazy or worsten your condition. I have heard positive and negative reports on the stimulator. It's a choice you need yo make. Try it and if it makes it worst, have it removed. It's worth a try.... Good luck!!

      no am not on any medication and not seeing doctor for any depression just fed up with the pain i will certainly try it as need to be pain free or at least better than am now. thank you for your reply
