Split tone in one ear

Hello,

i have been had a very distressing issue with my right ear for the past 4 years. When I am listening to music and it gets loud (especially with headphones) or when someone gets really loud i get this "split tone" in my right ear. I can hear everything perfectly well in both ears but I get this painful high pitched scratchy sound over what I am hearing. I would compare it to the sound of digital clipping. The weird thing is that sometimes I do not experience this issue. I cannot pin down what makes this worse. I also get ear pain far more frequently than the split tone, however the split tone is much more distressing to me than the ear pain. I have seen an audiologist and had perfect hearing in the hearing test. Any ideas of what this could be.
