Steroid injection for LS lesion?

After 12 week of clobetasol, I met with a doctor who agreed to do intralesional injection of the lesion to the left and right of my meatus. The one on the right - the pain disappeared! It is still red but doesn't hurt. On the right, he used what he had left (I asked after he began) and it trickled blood. That side swelled. Not painful but looked like a fat lip for 2 weeks.

It really worked well in the primary area he injected. Looking at my skin, he thought that topicals might not get through. I know to work in clob (and now Protopic) for 90 seconds etc. but if the skin won't absorb it, the ointment is just resting on the surface, correct?

I will ask for an injection at my 2 month followup for another problem area.

Any experience with these?

It doesn't eliminate the redness but it feels a lot better.

      I'd never heard of this shot being given to anyone with LS posting here. Did you bring this up in a previous post?

      I used Triamcinolone ointment initially, but it stopped working after about 6 months. I was not aware of this as my doc said she saw no signs of the disease where I was feeling a buzzing sensation. I went away and eventually increased the usage of the ointment per phone instructions from the doc. When I returned was told the LS had spread and was put on clobetasol ointment. I see you have also been using it topically.

      When they give a steroid injection is it typical that they go down a step in steroid strength? That article says they inject into the gluteal muscle, nothing about directly into the diseased area. Is you doc trying something new?

      No, it is not new. Kenalog is typically given in the gluteal muscle (systemic). I've had it before and it helped for a few weeks.

      The local injection is more direct and gets through the damaged and thickened skin. It's been 3 weeks so far and the place where I had constant pain no longer does! It is still red but if I have to live with redness without pain so be it. The doc also said redness is the last thing to go.

      Keep in mind that all studies of intralesional injections into LS spots are very small. Nevertheless, the doctor said there was little risk of atrophy and agreed to do it.

      The injection isn't painful. There might be some temporary swelling but that's it.

      You can have it done every 4 to 8 weeks.

      My derm did it but my primary care doctor also brought it up as an option and he is willing to do it when my appointments to the derm are far in the future.

      It's good you've found something that works for you; your case sounds extreme. I am trying to avoid steroids and am hoping to find someone competent (preferably an MD) who can administer PRP injections. This is proving to be difficult to find as it's considered experimental at this stage. As it's one's own plasma I don't see any risk, but medical people seem quite reluctant. Spa people are much more willing.

      Have you considered PRP? Don't remember...are you in the US?

      I am in the midwestern USA. Yes, this 10 months has been a nightmare. Just as I got function in the head of my penis (with help of injection), the shaft if now fiery red and distorted. Can't inject all of that!

      I have not got to the point of PRP. It seems my case is spreading very rapidly and don't know if they can PRP an entire penis?

      This forum is a godsend. I can't find any reports of men circumcised as babies who have genital LS. Frankly, other than Wee_Dugie (who is managing his after 8 years) I have not found any other man that I recall discuss this in the detail that women do.

      Look up Dr Runels in Alabama. He is giving the shots to men, but for different reasons. He might be interested in your case. Seems to like a challenge.

      Size, I think, but I don't understand how PRP would have that affect. Up till now he seemed to be concentrating on women's issues. Equal opportunity doctor, I guess. You can listen to him on his site.
