Still bleeding after birth, on the pill, what next?

Still bleeding at 13 weeks post partum.

I had emergency casarean of twin pregnancy. PPH of 1200ml.

  • Birth to 6 weeks - normal lochia
  • 6 weeks to 9 weeks - no bleeding
  • 9 weeks - first period - extremely heavy bleeding, gushing blood

Hospital advised me to start combined oral contraceptive pill immediately. I am nearly finishing my 2nd pack

I have been bleeding non stop since starting, sometimes bright red and small clots and sometimes dark brown and dry blood.

Ultrasound report below

What could be causing this?!

p.s I had totally normal regular periods prior to birth
