struggling to pee,should I reduce oxybutynin

hi, for 3 +years I've been on extended release 20mg oxybutynin because of bladder irritation. The doctors keep using words like urge incontinence and overactive bladder which simply does not fit, I can hold it but I feel I need to go all the time, it's unrelenting. Initially the oxybutynin helped,then I had a cystoscope and urethral dilation which set me right back. After several months of bladder training I've been fairly ok for approx 3 yrs and then out of the blue a month ago started feeling the urge all over again . I was told by 3 different Drs I had a bladder infection,then i didn't ,then I did. I have had 2 course of antibiotics but I do not feel this is an infection, the symptoms are not the same. Over the last week my bladder settled ,I feel due to some real effort with bladder training then I got up this morning feeling the need to go my pelvic muscles sore and tight, but couldn't with ease. There was loads to come out,the flow was very weak and I had to force it even though my muscles relaxed at this point . Then afterwards back to tight muscles and irritation, I feel because of straining. I had been finding if I held off after an hour or sometimes 3 the feeling would subside and when I did go I was emptying no problem . It was tough but forced myself not to go til the need subsided and then restarted,signalling to me that I did need to really go at this point. Why this is reoccurring with no sign of infection is beyond me, could it be that my dose is too high ,inhibiting the voiding and either incomplete or forced urination is behind the irritation. Can diet effect absorption, yesterday I started the Alternate day diet and ate very little, does this mean I've absorbed/utilised more or less oxybutynin? Any input would be greatly appreciated before I'm certifiable.
