Are you suffering from fistula or fissure? As u said you having Sentinel pile (Sentinel pile occurs mostly due to fissure) and then you also told you

are recovering from peri Anal abscess excision and drainage & unable to take sitz bath. Have you underwent surgery for fistula? Give proper description about your condition....

I had a chronic fissure diagnosed in December 2018 which has only just healed following unsuccessful botox

The abscess was excised and drained 7 weeks ago

In the last 3 days the tag has become inflamed and today is incredibly painful

There are no fissure type pains during bowel movements

You mean to say that only during sitting and walking you are having pain due to anal tag. Try to take some precautions for recovery such as go to washroom only if need is, avoid straining while bowel movements(that will prevent inflammation) , eat more fiber, drink plenty of water, take sitz bath 3_4 times a day, as sitz bath helps in healing wounds, you can apply anobliss cream or anometrogyl cream also but not for more time. Better is to go to gastroenterologist or Colo rectal surgeon he will tell you the exact cause with proctoscopy as anal tags don't cause much pain usually, and fissure can re occur anytime if you don't follow a proper habit and lifestyle. I think fissure may be the underlying problem.A gastroenterologist or Colo rectal surgeon will tell better. Wish you fine recovery.
