Testicle pain for 1.5 years

Posting this in case anybody can please point me in the right direction, so approximately 1,5 years ago my left testicle started hurting one morning, it got worse and worse during the day that I booked to see the doctor during the afternoon, by the time I saw the doc, I'd say the pain was 9/10, very tender and excruciating, the doc sent me straight to see a urologist at A&E.

Hours and hours later, the urologist ruled out torsion and did a scan, he said it was epididymitis. He gave me antibiotics and sent me on my way. The antibiotics did help along with painkillers and reduced the pain alot but not entirely. I haven't had an episode like that since but what I'm left with is a dull ache in the left testicle (the pain is at the back of the left testicle at the highest point of the testicle), I've had many doctors examine it and all agree its very tender as I can't stay still if they press on it. And every doctor had prescribed antibiotics since also, I've had roughly 9 courses.

I've even see a urologist that I waited 4 months for the appointment who also put me on antibiotics and discharged me. When the pain flares up, it's all I can think about, it's so distracting. It can get very achy and painful when I sit for too long but standing or walking, I barely ever feel it. It flares up after ejaculation.

My latest scans say I had a very small cyst on my right side but the left is clear. The doctor that performed my ultrasound said I have a possible varicoele on my left side, I believe she said it was 2.3mm whereas it needs to be 3mm apparently to be considered a varicoele. But 10 years ago, I had varicoele correction surgery due to testicular pain and had one bout of epididymitis in the last 10 years but that time one course of antibiotics treated it perfectly.

I'm just a bit lost and need some direction. I'm working with another doctor at the moment who is open to other treatments or referrals now that his 4 weeks of antibiotics didn't work.

    Daniel I have a very similar problem to you , I'm also left with is a dull ache in the left and right testicle, the pain is always at the back of the testicles at the highest point. My scans show 2.5mm and 3.1mm varicoceles both sides, also 2x cysts 2mm in size on the head of the epididymis. My pain is worst with certain jeans or trousers I wear, also sitting , standing, walking and ejaculation. Can I ask you, does your pain go down your legs into your feet? I'm currently waiting for a variceoclectomy.
