I have seen a PT for hip exercises.  They help.  I am with you with feeling pain while sitting.  When I sit, stretching out my legs in front of me help.  Sleeping without a pillow with your legs and arms straight also help.  My rheumatologist suggested an inversion table, which helps alleviate pressure on the spine.  Walking and yoga help me tremendously.  Good luck.....I know it can be depressing....

My AS isn't anywhere near as bad as yours sounds but I sometimes get the same hip pain (which has lasted a couple of years). My doctors have said that it is bursitis - inflammation of the hip bursa. I found it very hard to accept that I could develop new inflammation while I was taking anti-inflammatories! But anyway, two things have helped me - changing anti-inflammatories, after a month on the new ones things had definitely improved (but who knows if it would have improved anyway). The other thing that helps me is sleeping on my side with a big fat pillow - with my top leg and arm over it - this allows me to roll slightly forward keeping the bottom leg straight, taking the pressure off the bursa. Sometimes I also put the pillow behind me so I am rolled slightly backwards.

If these two things don't work for me next time the hip pain builds up to unbearable levels, I have heard that cortisone injections can have very good results and I might try that.

Good luck - the hip pain and no sleep is so draining.
