Thumping in right ear only when I'm falling asleep

my right ear has been thumping for a couple of years now. at first it was only once in awhile it became every night and only when sleeping. I had lots of docs tell me to live with it. I finally got an MRI that showed I had a Cholesteatoma. that's been fixed but I still have thumping and still only at night. it's erratic, sometimes fast then verrry slow other times. it is maddening, truly.
