Tips of index fingers twisted

The tips of my index fingers have become somewhat twisted over time. Not a big deal but now the left index finger has begun to shoot pain through it without notice. It also feels swollen a little but doesn't appear swollen. This is sort of critical as I play the guitar and must chord with my left hand. I work through it but would like to find out what's going on. I'm a young 71 and have no other health issues. I wonder if Tylenol would help?
