Sometime surgery is the way to go. I had TMJ surgery to the jaw bone to prevent it from "locking". Took all the pain away. Might be an alternative for you as well.

Physical therapy by a specialist knowledgeable in TMJ disorders helped me a lot. You really need to find the right therapist, though. Best to ask TMJ dentist or neurologist for a referral.

Also, did you just buy a mouth guard at the store, or is it a custom made orthotic that you have? I was referred to a dentist that specializes in TMJ disorders, and a custom orthotic was made for me. I was actually advised to wear it during the day as much as possible, in addition to at night. I didn't understand why, but it became clear to me that I was also clenching somewhat during the day as well. The orthotic keeps my jaw in a relaxed position during the day.

What symptoms did you have and has wearing the dental orthotic relieved them?
