today is one month since TURP

Hello friends. I am brand new to this site. And my highest regards to all of you in the UK! There may be sites like this in the US but this place seems to be very active and supportive. 

I had my TURP operation a month ago today. Right now there is no pain, bleeding, burning or other complications except for those associated with post TURP healing. I do have urinary incontinance and some lack of control. I must wear padded underwear in case I have a bit of leakage.I have read that all of these conditions will improve with time. I have had a very enlarged prostate for a couple of decades. But it really caught up with me about 6 weeks ago when I got a bad urinary tract infection. That set into motion this whole sequence of events that led to the TURP. I thought I would go home from the hospital and be just great. Not so. Complications seemed to come up one after another. Three days after the operation I had to go back into the hospital because of renewed bleeding, That took 4 days to get over. But when they removed the catheter I urinated normally for the first time in 6 weeks! It was great. All went well for a while after I got home. Several days ago I noticed that the exit hole of my urethra was small. I talked with my Dr about it and he gave me a self dilator to use "as needed". I also had some bleeding at the tip of my penis. When I urinate I have to try to hold back the flow for about 3 seconds or the on rush of fluid causes some pain at the tip. Stream is not bad bit is somewhat irratic and can turn from a stream to a spray and even change direction. I can only attribute this to possible scarring from the 8 or so catheters I had in me during the 6 week period that I went through all of this. Anyway, I see my URO tomorrow. My only fear is that they will try to put a cath back in me to do a void test. I can't see doing one just a month post TURP but who knows what these people will do. I am also afraid that they may just throw me over the self cathing. Seems like a short sighted solution when everything else is working great and just the front quarter inch or so of my penis is the issue... Anyway, I am sorry anout the lenghthy story but I am a bit scared and I know that some of you might have gone through similar issues. Thanks, Donald.

    Dear Donald,

    Sorry to hear of the troubles that you've been through. I am aware that 'they' call you back for a post-op check, but 4 weeks does sound a bt too soon. In the UK, I've seen it stated that the post-op check up is 12 weeks after the TURP operation, but my check up is mid-April, 2015 (23 weeks away!!! - oh the NHS!!!). I'm sure they will not catheterise you tomorrow. After passing urine (naturally), they will surely use an ultrasound to scan your bladder in order to determine if you're still retaining urine and if so, how much. I've also read that sometimes a urethra stretch is required, but I'm sure other forum members will know re this and give you more information.

      The urethra suffers a lot of trauma during any of the prostate procedures. I queeried my post op bleeding last year after my Thulium laser procedure. The urology nurse nurse said that the urethra swells up like a sponge and absorbs blood.

      In tweny years on prostate NG's and Forums I don't remember anyone say that their meatus (opening) was smaller after a TURP.

    Why did you have TURP rather than one of the laser options?

    Hi Donald

    Sorry to hear about your traumatic experience after the TURP operation and ongoing concerns. I had my operation on 15/10 and after various mistakes by the main hospital that did the operation and the A&E hospital I was submitted when I had the blackout I am still with a catheter after 1 month. The bag is looking clear and filling regularly, I do not appear to have had any infection. Hopefully this thursday the catheter will be removed and all will be OK with regular passage of urine with no pain or blood. Will keep you posted after thusday. I am having my post operation checks with urology consultant in early January which is 3 months after the operation. Kind regards Peter

    Hello Donald

    I am sorry to hear about your woes.  Like John has said,  I don't think from what you have described that it is likely you will be catheterised tomorrow.  So try not to worry. 

    Incidentally my own experience post a mini-TURP & BNI in early March is almost the same as yours as regards tip narrowing, with attendant flow rate and direction issues.  My Uro mentioned the word stricture and I was given lignocaine and a dilator. If I do not use this combination every day then the problems become acute in the manner which you describe.

    Apparently surgical stretching can be carried out and there is also a cutting technique that may afford some benefit.  Strictures however are pretty famous for re-forming over time.  So we are both setting out on a long road.  Post Radiotherapy for PC ( May /June ) I have now been diagnosed as also having,  an urethral tumour that was not even there in March this year and which seems likely to have been Rad Ther induced.  That might be lucky, as it might not be PC related and malignant.  I am having an op on 3/12.

    I do hope your med team will get your problem sorted out well and quickly.
