Too embarrassed to see dr but am worried

I don't know if I'm writing this on the right board, apologies if this is in the wrong place. I'm a bit worried because I have something wrong when I go to the toilet but can't find any info online about what it might be. My dr's surgery has no female dr and I really don't want to go and see him about this.

When I either sit too long on the pot or strain, when I come to clean up there is something behind the rectum that "clunks" back up into place, and it really does clunk when it goes back up. It's as if something internally anatomical drops. It isn't like a haemorrohoid or anything like that because it isn't actually where the rectum is, it's further back. Once when it popped back I got a sharp pain hit me around the front where the inside thigh meets the torso/pubic region.

I've looked all over the net and from my own knowledge of anatomy, I haven't the foggiest what is going on or if it's something I need to worry about. I've been doing Kelgels in case it's the peritoneum that's just weak , but I'm stumped as to what is going on and if it's something that strengthing the muscles will fix.
