Transperineal laser ablation of the prostate. (TPLA)

Wondering how TPLA procedure has worked for people. Know it's not been around long, but surprised search didn't find the topic on this site.

I had PAE about 10 years ago, was great for a few months but then failed, and couldn't redo due to tortuous blood vessels (would take too long on table). Prostate volume: before PAE 163ml; 3month 136ml; 6month 156ml.

On finasteride since prostate volume reached 205ml about 6 months ago. Condition improved, most days fine and just 1 or 2 visits a night, but still need ISC (mainly for occasional arrested flow).

Don't want to stay on finesteride long term.

Offered greenlight, but think too risky. So looking at other minimally invasive procedures.

Is there a upper size limit for TPLA?

See one study trying up to 150ml, hoping mine might make it to this with finesteride .

    For such a large size of prostate HoLEP probably is a choice if one does not care retrograde ejaculation very much but the lowest regrowth rate.
