TSH 21 T4 normal range?

Hi looking for some guidance on my latest thyroid bloods pls. TSH 21 (0.38-5.33) free T4 11 (7.9-14.4). I'm on 75mg levothyroxine it was reduced fromm 100 a few months ago as my t4 was just over range. Other results B12 259 (133-675) folate 11.3 (3.10-19.9) ferratin 31 (11-306.8). I don't understand why my TSH is so high if T4 is in range anyone any ideas before I speak to Gp? Could this b a conversion problem? Also should I be doing something about vitamin Iron levels? Many thanks
