type2, no metforming, use insulin only as-needed?

I just talked to someone with a combination of condition and treatment I have not seen before.  Type2 diagnosis, *not* given metformin or any other drugs - except for short-acting insulin to use as-needed.

Says his average is now about 110-115 pre-meal, controlled mostly by diet and exercise - and an occasional injection when he eats too much sugar and gets a high post-meal reading.  FWIW, this is in the US.

Anybody ever hear of this diagnosis + protocol before?

    Hi jx,

    History - diagnosed as type 2 with HbA1C of 14.8% and BG of 498.

    My regimen is almost the same as your friend - the difference is that I use a medium-acting insulin as needed (usually 1 - 3 times per week on average, 10 units each time). I used to take a basal insulin daily as a form of early insulin therapy.

    I don't use any other medication. Fasting BG is reasonably stable at 109-116mg/dL.  HbA1C now 6.4%.

    This was against my doctor's advice who had prescribed the usual (Metformin, DPP-4 inhibitors,

    GLP-1 receptor agonists etc). Once I understood the risks of routinely taking these drugs I just decided to go my own way, and it seems to work fine for me. I get BG spikes after meals but they are rarely over 180mg/dL and usually drop back after 2-3 hours to <120.
