Ultrasound guided cortisone shot. First one not working??

Very active 80 year old man. Have had this moderate to severe OA for a while. First treated by Ortho Dr as Bursitis with exercise. Some relief but started getting worse. Prescribed supervised PT with Tylenol. Got worse and gave a local shot in the hip. Just stuck it in. Ne relief. Gave second shot, same way, 2 weeks later with no real results. Did MRI and diagnosed moderate to severe OA. Prescribed Tramadol, as needed, more PT.
Still getting worse.
Moved and got new Hip Dr who gave me computer guided shot. Total relief second day only then back to a somewhat reduced pain all the time. Tramadol and Tylenol give some relief.
Its been 5 days. Should I wait longer before contacting DR and will he give me second shot to try relief?
Probable replacement in the near future.

I would say don't bother, if one shot didn't work then two won't work any better. When works it usually lasts a few months. If you were to get relief from the guided shot it would have happened by now. If the hip has OA it will gradually get worse. I was recommended a hip replacement in summer 2019, but was persuaded to try a guided shot which didn't work at all after the anaesthetic wore off after 24 hours. And here I am in December 2020 still waiting. I am in the UK BTW but my advice still stands. You say you have had the pain for a while, how long is that and what do the x-rays show? I would consider having the THR now while you are a 'fit 80 year old', The results will be better than on a 'not-so-fit 85 year old'. I am 69 and had one hip replaced in 2015 and the pain before the operation wasn't anything like as bad as the pain I have now. Good luck!

As Howeller says, pack in the cortisone shots. There has been some research saying that cortisone injections are not a good idea long term. I actually take corticosteroids and the surgeon said they had eaten away at the cartilage so I needed hip replacement. Sadly the hip will slowly get worse so it is worth considering a replacement. I am sure you will never regret it, it is a very successful operation.
