Unable to determine the cause of Iron deficiency anemia

Hi all, 

I have been diagnosed Iron Deficiency Anemia (IDA) since 5 years ago. However it got worsen in last year. My ferritin dropped to 10, Iron level dropped to 18 and heamoglobin level dropped to 8.1. The hospital did the blood transfusion for me and referred to hematologist. However, they did all the tests (i.e. endoscopy, colonoscopy, test for h.pylori) but the result shown negative. I tried to consume more red meat and high iron food and also avoid coffee intake. However there is no improvement on my ferritin nor iron level, 

Due to I experienced migraine when took the tablet iron pill and diarrhea when took the liquid iron. Hence, the specialist given me the iron infusion in last year july. After the iron infusion, my heamoglobin level when up to 12 and the ferritin is 129. I continue my life with iron intake via food. 

However, 8 months later, i start to experience fatigue and short breath again. Did the blood test last month, result shown everything dropped again. heamoglobin level is 10, ferritin is 10 and iron is 20. When back to my specialist, did another 500mg iron infusion. He said have no idea why my ferritin level dropped and it is abnormal. He have no solution other than do the iron infusion as and when it drop again as all the tests shown result is normal. 

As my iron level drop, I tend to get infection frequently especial for my sinusitis. Through this 2 years, my weight also gained about 18kgs.

Wonder if anyone can give me advice how should solve my issue.

    Hi Irene. I presume they have checked you folate, b12 and vitamin d levels?

      Hi Jenny. Yes, they did checked my folate (fluctuated but in normal range) and B12 level is in the normal range. But they didn't check on Vit D. 

      My hematologist have give up to further investigate for the root cause. He only said visit him again for Iron IV when my ferritin drop again. I feel so down and frustrated.
