Undiagnosed/Unexplained Burning Rectal Pain for 2 years

Hello,
I am a 27 years old male.
My pain started April 2018 (2 years ago).
The pain comes when i have a bowel movement especially if my stools are a bit on the watery side.
The pain is localised and is a an extreme burning pain that can last for hours after any BM.
The pain started suddenly and i recall using glycerin suppositories due to constipation before the pain started.
In the past two years I have done an MRI, MR Defecography( it showed a bit of paradoxical contractions), Anal manometry, colonoscopy, sigmoidoscopy and anoscopy and biopsies were taken. There is still no explanation. I saw an anorectal surgeon last year and he said I had fissures so I underwent a Lateral Internal Sphincterotomy. The pain only decreases a bit faster after the BM but its still bad. I went back to my anorectal surgeon and then he started speculating that I had Solitary Rectal Ulcer syndrome ( I have no rectal bleeding or mucus discharge) and he told me to do biofeedback which I did and had very little effect on me. He suggested Ulcer excision but I saw 3 other doctors that they saw no ulcer or anything else but just internal hemmorhoids.
This has taken a mental toll on me as I live with this pain every day with no diagnosis.
It is also worth noting that when ever I did enemas for colonoscopy preparations that salinity made my pain so intense that I had to take pain killers. As if pouring alcohol on a wound.

and I hope I can get an answer.

Hi, I am getting a similar pain to you....my life has been hell for 6 months now, but the last 3 months its been unbearable and i wanted to give up at one stage!! I know i have a hemorrhoid, and it itches and hurts like hell but i'm always told hemorrhoids don't do this? Now i have a constant burning in and around the anus/rectum.....i also itch like hell..i cannot control the itching and its everywhere, not just the hemorrhoid. The burning is awful and hurts so much, and when i pass a stool it severely burns to the stage where i cry out, and this burning/stinging goes on for hours afterwards. There is no relief....i walk around my house all day and night because the burning/stinging/itching is just not bearable. I have even taken myself off to hospital but they didn't really help, one dr said its i had a urnine infection and another said it was just a hemorrhoid. I have been to my local dr 5 times now and one thing he has said that really makes sense to me, and could also be what you have...is that they feel i have Proctits. I also do not have any mucas or bleeding in my stools, but they said it is still proctitis, and that does seem to go along with many of my sypmtoms. All i know is that i cannot carry on living like this, life is just hell on earth and i can't do anything but lay flat on my back and cry with the pain. Look up Proctitis and see what you think.

I am really sorry to see you go through that.
i have tried previously mesalamine suppositories and they helped with the pain which was an indication there is an inflamamtion.
I did a self digital rectal exam to understand where the pain is and it seems lower down in the anal canal. And as you know if there is pain its probably around the dentate line where we start feeling pain.
Having said that I want you to google the following terms - Anusitis, Anitis, Cryptitis, Papillitis. Anal crypt inflamation.
So basically its like proctitis but of the anal canal.
Another update is that I recently went for an examination under anesthesia and the doctor performed a hemorrhoidectomy on my anterior internal hemorrhoid and he said it was my cause of pain. He sent it for biopsy and the report said chronic inflammation and fibrosis.
i did my surgery 4 weeks ago so am still recovering from the procedure which is unfortunately very painful. I still feel oain where i originally had it but my doctor said to wait since he excised that are so it is normal to feel pain.
I know this is very mentally draining but try to be patient and I suggest an Anoscope and EUA if you have insurance.
Ill keep you posted if I feel better in 4 weeks.
I also highly recommened sitz baths after you have a BM itll decrease the pain tremendously and shorten the period.

I hope your pain decreases real quickly, it is an awful place to be in pain as we cannot soothe it or rub it to help us and its such an uncomfortable thing to talk about to people.
I am not sure exactly what i have going on but i know i'm in pure agony. If i could control the severe itching it would really help, that is what trigers it all off. There is only 1 hemorrhoid thats been seen, but i can actually feel lots of other very small lumps and bumps, some of them are so tiny like pin prick size and feel almost 'sharp' ....i find this really strange! Its obvious they are not all hemorrhoids. The whole area itches, the rectum, anus, perineum, and higher up at the back, this itch turns into a terrible stinging, like hot pin pricks and the pain is awful. As soon as i pass a motion everything just goes beserk, its like because the motion touched the skin its all screaming out to be scratched but once you start to scratch you can't stop, you really can't....and the ithcing goes deep up inside the anus ( i'm told hemorrhoid internally cause no symptoms?) So i use water wipes or anusol wipes to relieve the itching as even the softed toilet paper has made me bleed, i apply hemmoroid cream all day and night to numb the whole area inside and out, it is the only way i can get through each day with out crying. Once i have rubbed to relieve the intense ithcing i end up with a deep throbbing inside and out and pain and soreness, i live like this every day of my life and Dr's say they just don't know what is wrong, because there is actually nothing hardly to physically see apart from split skin and a sinlge hemorrhoid. I cannot even touch down there as i'm so insanely sore, even the softed toilet paper is agony and i bleed so there is no way i'm going to be able to let anyone do anything about my hemorrhoid? This is a miserable way to live, i wish i could get some relief. I am now on steroids so hoping they will help me.
