unintentional weight loss

My 17 yo son lost stone and half over 4 months without trying, initially when he was eating he would get full just after a few bites but after a couple of months that became better and eating lots more again but was still losing no matter what and he can't gain any of it back, he's under his weight from quite a bit, his bmi is under 18, he's very tired and started to feel low because of it. He gets headaches and his hair has become very thin. He's had lots of blood tests, he had folate deficiency and very low range b12 of 220 and ferritin 53. I think his thyroid looks swollen but again doctor said that he has prominent cartlidge. Anyone have any ideas? Could his folate, b12 and ferritin be to blame? Thank you
