Update on my failed Urolift

Had my Urolift back in March and it made my flow worse. My Urologist suggested a Bipolar TURP and I said no because I still enjoy sex and knew that some TURP procedures can result in retro-ejaculation (RE). I had told this doctor I was OK with my flow and thought my problem was OAB. He prescribed Mybetriq and it helped me a lot for about six months then stopped working. My flow tests showed I had a problem even though I told him I was urinating fine. I trusted him with doing a Urolift thinking it might help but it made me worse. He suggested I get a second opinion and I took his advice.
I went to another Urologist (he uses the Rezum procedure) on 8/12/2019 and showed him my medical records and asked him about removing the Urolift clips inside my Urethra. He that might make things worse and said I had more than one problem and that it made it difficult to treat. He said that I have an over-active bladder and now some blockage from the Urolift and a small be enlarged prostate. He recommended that we try and treat one at a time or both at the same time. He prescribed Tolterodine 4mg cap (Detrol) for my OAB and Dutasteride (Avodart) for my prostate. I decided to try one at a time since if I had side effects I would know which one is causing it, so I decided to work on my OAB with Tolterdine since I suspected that was a problem I have had for many decades. Since Mybetriq helped me a few years ago maybe Tolterdine might help. I can live with the flow from the failed Urolift for now and later might try Dutasteride to see if it might help me more,
For now my second opinion Urologist suggested trying medicine for 6 weeks or more and if that did not help then he would look at other options like Rezume. I will update this in the future.

I'm in need of prostate enlargment I've seen two urologists one wans to do Urolift the other says its only 50% effective and he want's to fo greenlight laser. I'm very confused ! My prostate is 40 grams, do no huge, but I'm not emptying my bladder any thought? yes, I've taken FloMax for two years and not working

Maybe an ablation like Re zume would help. Otherwise I'd go for greenlight

I keep reading in various places urolift can fail. but I'm not finding how, in what way, it failed for everyone. In my case, when I had a second opinion done less than a year after having my urolift put in, he told me he could not see any urolift in there. He said that if he did not read in my chart that I had it performed, he would have assumed it was a virgin prostate. I assume that means it ripped through and is embedded inside? Does that ever happen normally? When they say the procedure fails, what is a normal failure? I'm trying to figure out if they just did it wrong. The doctor that did mine completely blew me off when I said something wasn't right, and told me that you might just have to catheter instead then. He never even offered other options.

I had my urolift on August 2nd 2022 so today 8/21/2022 I'm just short of 3 weeks and during first 2 weeks after procedure I tried to urinate while not on my Meds (8 MG of Doxazosin Mesylate) and (5MG of Finasteride) my goal was to be able to get off the meds as I was Maxed out on the allowable Dose. So i was able to Urinate ok during first 24 hours but then very poorly after that, so per doctors office instructions I went back on Meds until I had my post Urolift Procedure Doctor visit where per their instructions was still on Meds at visit they took a Velocity test and then Ultra Sound of Bladder both of which were good also noted was my Bladder emptied more then before Urolift under Same conditions meaning i was on same Dose of Meds, so Doctor wants me again to try and slowly get off Meds which I tried and was able to feel ok urinating for almost 48 hours then Had to take Meds again which Dramatically helped me so now I'll try to cut in half the dose hopefully everything works out in the end. I'll keep you updated. If you or anyone have any information that might be useful please update. Have a Great Day

They can send rocket ships to the moon and another one to take photos of Mars, but they can't figure out one damn way to treat BPH successfully, with no side effects, like extreme pain and irreversible impotence. So damn sad!

I had the Urolift procedure done about five months ago and thus far I'm dissatisfied with the results. While waiting for the appointment, I did a lot of research and discovered the Urolift installation devise had been revised and improved upon. the original is a UL400, which was replaced a couple of years ago with a UL2 model. I watched several videos by different urologists who extolled the benefits of the new UL2 devise and how it was improved in several different ways.
When we had our final Q&A meeting with the Dr. prior to having the procedure, I asked him if he was using the UL 400 or the UL2. I didn't get much of a response and I now believe he had no idea what I was talking about. I regret now pushing him on the issue but I assumed since it was a modern, popular medical facility, surely they would be using the latest technology. It only made sense since it was made by the same company, why would they order the older version?
On the day of the procedure I was prepared and wheeled into the room where the procedure was ready to take place, and the first thing I see is a package containing the old UL400..! I would never have continued had I known, but by that time I was already totally committed. I rationalized that the older model had been successful, just not as up to date as I might have wanted, so we proceeded.
I'm sure using the older technology had nothing to do with my lack of success, but it makes me question the Dr. who was so out of touch, maybe he wasn't the right guy for the job.
The procedure went well, I was knocked out so missed out on all of the pain, and recovery was not a problem after a few days.
As time went on, I wasn't seeing any progress, I waited a few weeks and wasn't seeing any positive results. The after check up a week later, I was assured it takes time, so come back in three months and everything should be fine.
I saw no results at all until the eight week mark, and then had a few nights where I wasn't getting up 3-4 times so I thought I was on the way. I still had trouble starting, and the stream never really improved. but if I could get through a night with out getting up 3-4-5 time, I would be happy.
Then came the inconsistency, one night I slept for seven hours and was delighted. Next night I was up 3-4 times, but I stayed patient thinking it would improve. It hasn't. After 4-5 months, I still will have a good night, then a week of getting up constantly. I have far more bad nights than good night. I have just enough good nights to tease me a little and make me think it will improve.
It makes no sense to me, if the blockage has been removed, it should work every time, but it doesn't. I still go too frequently during the daytime but can live with that, I just want to sleep at night.!
Today I looked up how long it should take to see results and found this;
When the pressure of the prostate on the urethra is mechanically reduced with UroLift implants, the urinary stream almost immediately improves. In fact, some patients report a dramatic improvement – or even reversal – of many of their symptoms just 1 – 2 days after treatment. On average, however, most patients require a few weeks for the full effects of UroLift to become apparent.
Should only take a few weeks max, right? So after my four-five months of waiting looks to me like the procedure was a complete failure. I'm unhappy and discouraged.
I've lost all faith in the Dr. and I'm not willing to undergo any of the options other than trying medication. I shall continue to play the game until my next check up at the six month mark, but I'm not holding out much hope..

I had the Urolift procedure in June 2021. No change to symptoms at all apart from near 'urgency incontinence'. A recent cystoscopy revealed that one end of one of the four implants used was in my bladder (rare occurrence) which has started to 'crust' and a stone has formed which has now been lasered and the rogue implant removed.
Now looking at Rezum to see if that will help, however I have been informed that when Urolift has failed, the next line of treatment is a TURP, which because of the possible side effects, impotence, retrograde ejaculation, incontinence - put me off. Has anyone had another treatment after Urolift? If so which?

You can do aquablation after urolift. The doctor should be able to remove the clips when they do the cleanup procedure after the aquablation water jet has removed the prostate tissue. I would find someone really experienced with this procedure. If all goes well, you should be able to maintain ejaculation as well.

PVP is also possible after Urolift - my Urolift failed as well and that was my next port of call. PVP not a success either although understand it works well for many. Ended up with a TURP, including because not all of the tissue was removed with PVP.
