Uric acid levels aka serum urate

Hi am posting the following as i want to know about others experiences in reducing uric acid levels with allopurinol ( dosage ) and timescales.

I would ask that people refrain from borderline personal attacks i have seen on other gout posts. You know who you are.

13th december level was 509... under attack!

13th jan 431 had been on 100 mg for 1 month

23rd Feb 330 had been on 300 mg for 1 month.

What are others experiences and can I realistically hope to be down to 0 by the end of the year?

Remember, good natured humor ok, Dudley do right attacks are not...

    I'm on 300mg Allopurinol every day but still had a

    bout of gout lasting 14 days. I found drinking plenty of water helps to flush away the Uric acid.

    I've been taking 300 of allopurinol and I also take two tart cherry pills from GNC. I have to go back for blood work but I have not had a flare up in a while comma although I did have many beers this past Friday and my right foot is hurting today. I've taken the indomethacin this morning and it helped I'm about to take the other period going back for blood work soon to check my liver function and obviously my uric acid levels. GOUT sucks good luck

    Hi Roger

    Your question is not clear. Do you mean get your blood urate down to zero (answer no and not desirable). Will you get your allopurinol down to zero (extremely unlikely). It seems to me that with blood urate at 330 you are at the right kind of level (low end of normal).

    You need to carry on at 300mg of allopurinol (standard dose when trying to flush tissues of monosodium urate), until you haven't had an attack for a long time and your blood urate is low. At that point many patients go into low maintenance dose of 100mg.

    However be aware that 85% of your blood urate comes from your own body, not food. Therefore the best thing you can do is reduce your body size to slim (assuming you are overweight). There is also a strong association between gout and fatty liver. If you lose weight fatty liver will heal and that will in turn help your gout.

      Hi

      My intention was to get it down further to 0 with the view that any crystals are then no more.

      I am losing weight 1stone so far. More to go.

      The conflicting information would suggest 360 or below is equal to "5" and is considered safe or"

      Why could i not further reduce my weight and urate level and get a lower urate reading?

      Btw. We all know this is different for everyone, in the first month i was eating 200gs of blueberries or cherries. Combined with the 100 mg meaning i droped 80 in the first month only 20 less than i did in the following 5 weks on 300 mg.

      So perhaps the natural things do work people!!!

      Hi Roger

      There will be variation in your blood urate levels - they change constantly depending for example in what you ate, whether you have an injury or an illness at the time. Two other factors. If you are losing weight you are effectively eating body fat - this metabolism also adds uric acid. Finally urate crystals are protected from attack by white blood cells by a layer of protein covering them. So as you maintain low blood urate the body takes off the protein layer exposing the crystals which then dissolve into the blood to be removed by kidneys. This process is not only what can trigger gout but also will cause variation in blood urate.

      You won't get blood urate down to zero because the body constantly produces the stuff from

      Catabolism and digestion of food.

      5 is normal for a normal male. It is just about acceptable for someone who suffers from gout. However lower would be better to remove uric acid stores and to reabsorb toohi and deposits in the veins.

    Approx 10 years ago I had my first attack of gout, in the intervening years I had two others. All three of these attacks I just put up with and took Naproxen ( I now know that this medicine to have been a complete waste of time ). November 2016 I had an attack of gout in both feet - I had steadfastly been refusing to take allopurinol - I realised this could not go on and with an initial dose of cholcicine to combat the worst of the gout I started allopurinol on a daily basis. Apart from two very mild attacks of gout, that I was warned could occur, I have been gout free every since and my uric acid levels are of an acceptable level. I suggest to you that you just take the allopurinol and hope for the best, ignore the time consuming "uric acid level game" and get on with your life

      I am not sure what you mean by the uric acid game. It could be that in your case your level of UA is marginal and you can afford to ignore it. It is a fact that if UA has been high for a long time the patient has substantial stores of UA crystals throughout the body which lead to heart kidney disease and certain cancers ten years down the road. I suspect that most people ignore them, and that leads to substantial ill health.

      Compared to individuals without gout, we found that individuals with gout died earlier and experienced a *42% higher risk of death*. Similarly, individuals with gout experienced a higher risk of dying from cardiovascular disease with a 58% higher risk of cardiovascular death.

      First of I would like to state my age. I'm 66 and in remarkably good health considering my working history that includes The steel industry, that I left with a great deal of help from a certain Tory leader that shall remain nameless, after retraining I've spent the last 20 years in the building trade. I left both of these trade with a quite considerable amount of wear and tear. The onset of Gout has not helped and was not foreseen. I spent quite of lot of time and energy doing what you are doing and looking for some kind of answer. There is no one simple remedy and I have found that, for me, allopurinol is the remedy for me, until I suffer another attack and then I'm back to square one, hopefully that will not occur!

      I won't be responding to anymore discussions - in fact I'm going to cancel my account with this website - all it does is create more concern, without solving anything.

      The fact is if you have got Gout "YOU HAVE GOT GOUT" getting involved with the uric acid percentage game cures nothing.

      Ok.

      I intended this forum to be about the management of gout.

      As such I want to gauge opinion as to what i can expect.

      So i respect others right to just take Allopurinol and forget it, but I want to hear from others who are analytical in there approach.

      For example has anyone managed to get the level down taken a manangement dose (100 mg) and been totally free of attack??

      Obviously this is where I would like to be.

      I mismanaged my condition by ignoring it for long enough. NOW is the time to manage the condition and my expectations.
