Urine leaking

I've had a lot of trouble with my bladder over the years. when i was younger, i would press in my bladder when it was full. i don't know why but it did get painful when i would do it for too long. i often have a feeling now where when i pee it feels like not everything comes out and like i can't get the muscles to close back up. I'm also struggling to tell when my bladder is full. it doesn't even register until its bursting and painful. most days, my underwear is damp with urine and its making me self conscious. I've started carrying around spare underwear with me just in case. Could somebody please shed some light on this
