UTI?

I’m hoping someone can offer me some insight. Apologies in advance for the long post.
On 14th December I went to the doctor with a suspected UTI. She did the dip test and said it was positive and gave me a prescription for amoxicillin. My symptoms improved then returned after Christmas so I went back, doctor did another dip test which again was positive so I was given cipro and my urine was sent off to the laboratory.
The cipro did nothing to improve symptoms so I treated the UTI myself with D-Mannose. I thought it had cleared up (I felt almost 100% better) but then the symptoms returned so I went back to the doctor. She checked my lab results which showed unlikely infection despite the positive dip tests. She also did another dip test which was negative. The doctor referred me for an ultrasound of my ovaries and uterus, I’m awaiting an appointment.
But this morning when I urinated there was bright red blood when I wiped. None in my urine but a small amount the tissue.
My symptoms are frequent urination but not a lot coming out, pelvic and back pain. I also have some hip pain along with tiredness.
It’s worth mentioning that I had exactly the same thing happen a year ago, three lots of antibiotics didn’t work ended up using d-mannose and I’ve been fine until now. I also had an abdominal ultrasound at the time which was normal but the doctor didn’t check my ovaries and uterus.
The blood when I wiped has frightened me. I intend to get another appointment tomorrow but has anyone had a similar issue with a UTI? Thanks for reading.

have they done blood work this sounds like kidneys to me. in some cases the test is negative.

Thanks for your response, no specific blood test for my kidneys but i did have a routine blood test to check my Thyroid and iron levels as I have an underactive thyroid. I will request one at my next appointment though.

hi could i ask when your last smear was?
i only ask as i went through the same last year and after my smear it turned out i had cin3 precancerous cells which did turn out to be cancer.
i went through the same with 3 lots of antibiotics etc too. i had bleeding and pelvic pain and weed more than normal.
sorry don't want to scare you but worth checking as docs don't really offer an alternative. you can always push for a pelvic ultrasound too

Hi thanks for responding, I am up to date with my smear tests my I believe my last one was around two years ago so not very recent. The scan I'm waiting for is a pelvic ultrasound so we'll see what they find. Thanks again.

Hi,
When I was younger I used to get really bad cystitis and passing blood became the norm (so very painful) doc came to the house once as I was in too much pain to sit in the surgery, she gave me the little bottle for a sample and it was completely red, she said "Have you got a period" I said no it always comes out like that. Severe inflammation of the bladder is agony the pass but nothing to worry about. incidentally if you are sexually active don't use condoms they will rub.

Why not ask your doctor IN WRITING if there is any reason not to have a CT scan?Is the service trying to save costs? If your GP is not allowed to endorse a CT scan why not a referral? Your health is the most precious thing we have. You are quite entitled to ask pertinent questions about your situation and a letter is advice to anyone reading it that you are concerned.

No, don't have a CT scan, I have not felt well since mine, a massive dose of radiation for what is probably something simple

As I said a referral seems to be the most constructive way forward. It must also be said that a confirmation be asked for in respect of past medical history. It has been realized lately that some doctors ( too many to make any sense of the matter which is unbelievable but true) do not refer to medical history.
One of my medical relatives states that they are instructed that the patient should tell the doctor about previous medical history........ I and others find that totally unacceptable.
Good quality urinalysis may find an infection but is by no means 100% guaranteed if the colony of bacteria is small.
Millions of successful CT scans have saved millions of lives all over the world so if I were the person knocking CT scans I would think again about denigrating that useful tool in the modern array of disease prevention.
The scans also proved in a massive percentage of scans that there is nothing sinister going on.

I agree with both responses.
It could be kidney related such as kidney stones. They can cause blood in urine. I had UTI symptoms for months, but turned out to be a stone that required lithotripsy.
The other suggestion about the blood being from vagina is also worth exploring. I have learned that UTI symptoms are similar to vaginitis which can be easily treated with OTC creams.

Not really to do with your question but I would just say to be careful if you are prescribed cipro again in the near future (or another fluoroquinolone). Not only is there a risk of getting resistant bacteria but the effects of the FQs are cumulative so, although you didn't have any problems this time you may well get side effects such as those listed in the packet leaflet with the next course. You don't say if you had Cipro a year ago but if you did, you should be extra careful. The FDA have made several warnings about the FQs recently and the EMA (European equivalent) are also issuing new warnings in the next month.
Side effects to cipro are no joke, they can be permanent and disabling, there are threads on this forum about it under fluoroquinolone antibiotics.

Posting for anyone who might read this with the same symptoms (I am the OP) I had the ultrasound scan which was normal, the cultured samples came back negative for infection so I just lived with the symptoms. Recently the symptoms got worse so i went back to the doctor who performed a pelvic exam on my request and it turns out i have a prolapsed bladder which is what has been causing the discomfort, back ache and frequent urination. Ive started pelvic floor exercises to try to prevent it from getting worse.
