Vas string came out 3 years post vasectomy

we can't seem to find answers anywhere and my husband's urologist won't see him because he does not deal with vasectomies anymore he sent us a certified letter stating "he is not held responsible for any unexpected pregnancies" a few years ago which we thought was strange but maybe that is normal with vasectomies and most docs do, just know when we reached out about this problem we were turned away. He had a large lump that kept growing on one side right over the vasectomy scar that developed about 3 years post vasectomy. His primary doc sent him for ultrasound. "Nothing to worry about" is all he was told, a few months later he sees something coming out and was able to remove it thru the hole from the vasectomy. It was for sure the tie used to tie the vas on that side, had a small knot/loop in it. Anyone ever hear of the tie coming off? I think our biggest concern is what this means going forward..thanks in advance we can't find anything about this anywhere
