Venlafaxine withdrawal itching on 37.5

Over the past 5 months I've been weaning from 150mg Venlafaxine and managed to get down to 37.5mg. I need some help of off someone as my GP is useless! How do I wean myself over so many weeks of the 37.5mg please ?? Infact for the past 2 months the 37.5mg I've been cutting that one tablet in half and taking that half of the tablet. I've tried recently to even take that half 9f a half every other night and all I do is itch like crazy. Please don't go into a percentage of a dosage LOL because I don't understand
