Very bad abdominal pain for past few months

Hi,

I'm 18, female, and overweight. When my pain first started it was upper right quadrant, but anymore it's upper right, upper right side(near ribs), and around colon. I get really bad "attacks" at night and after I'm eating, which made my doctor think gallbladder. However, blood tests, X-ray, ultrasound, and HIDA scan were all normal.

Anymore the pain has become unbearable and I have a week of severe constipation followed by weeks of diarrhea where I can't keep anything in and can't control it. That cycle keeps repeating. I've also gotten mouth sores and nausea.

Could it still be gallbladder or is it possibly something else?

P.S. the first time I had the pain really bad was after the one chip challenge(hottest chip in the world)

It sounds like something else. If your gallbladder scans were normal, your doctor has ruled out gallbladder problems.

Since you are  having bowel habit change and food reactions, it sounds more like a bowel complaint. Two things spring to mind: IBS or IBD.  Some forms of IBD, such as Crohns can cause ulcerations in the upper digestive tract; mouth ulcers could be a sign. A pill camera can detect this.  If Crohns is in your bowel, it can be detected by a fecal calprotectin test and a colonoscopy to confirm it.  However, if this was the case, you would also have bleeding and weight loss.

If it is IBS, nausea, shifting stomach pain, bowel habit change and food intolerance are symptoms.   I would go back to your doctor and ask for more tests.  In the meantime, stick to bland foods and avoid anything rich, spicy and fatty. Try a food diary to identify food triggers.  Sounds like  the spicy chip, triggered your symptoms.

I have an appointment with my GI later this week

I forgot to add that mouth ulcers may also have been caused by spicy foods irritating your mouth.  Hope you get answers.

Hi , you wouldn’t necessarily have loose stools, blood or weigh loss with IBD. It’s actually very common to not.  My mother had crohns and never bled, I have 3 friends with IBD who are healthy in weight and suffer chronic constipation. It’s a common symptom to have some the weight loss etc but not a definite sign. Please be aware of advising people this, they need to see a GI professional

I am quite aware that anyone with GI problems should see a GI specialist and I did not suggest to you that you should not see one, far from it!  Most of us on this site are not doctors and are vulnerable to making wrong assumptions about particular conditions because it is easy to generalise and mistakenly think that everyone is the same.   I am sorry I gave the wrong impression, but I am only human after all.

Just please be aware , this is the 4th time I’ve seen you give out incorrect information .

I’ve had this for years, drives me mad. Had every test under the sun. It’s gas trapped in the ascending/descending colon. Although it doesn’t feel spasmodic like IBS it is. It’s very painful and caused my food intolerances . Good luck and I hope you get better soon
