Weeks w/out sleep: 20 years of chronic insomnia

For the last 20 years I have battled chronic insomnia. I take a very scary cocktail of meds and over the years my sleep has gone thru cycles. I would sleep okay for a few weeks, then out of nowhere I would stop sleeping for three to five days. I would have horrible pain all over my body, and fatigue which prevented me from functioning. My doctor would switch my meds up occasionally and sometimes it would help temporarily. I founded and ran a successful company thru this 20 year period, but the problem go so bad I could no longer hold the company together. My life's work was lost, and my wife who i met in high school left me. I found a new line of work, but its slow in getting started and just as i was making progress my insomnia suddenly got worse than its ever been in my life, and now I never want to eat and i dont know why. I have reached the point where ive become apmost completely dysfunctional and everyone around me is p****d off and frustrated because they are worried and dont know what to do. My wife and i are back together and rebuilt our relationship, but now things are getting tense. My wife is frustrated about how my limitations hold our family back from doing things as a family, it makes having friends difficult and financially it's a major problem. I want to go to a sleep center of some kind where hopefully they can evaluate my condition and get to the bottom of it. Any advice would be greatly appreciated.
