    Subject: Consultation and advice for Sushila Devi Goel suffering from Wegner’s GPA

    Respected Sir,

    This is to inform you that the patient was diagnosed with Wegener's granulomatosis with polyangiitis(GPA) and the treatment was being given in Apollo Bangalore. She was discharged on 1st December. I am attaching the discharge summary along with. She was improving lately. On 20th of December the 3rd dose of cyclophosphamide (1ooo mg) was given to her after which the conditions started to decline. The BP was fluctuating usually low with fever between 100°- 102°Fahrenheit, Urine output around 500 mL along with drowsiness. On 23rd December the creatinine level was 4.5 and on 24th suddenly raised to 5.4 . She was admitted on 24th night and was unconscious too . The urea level was 400 . Doctor told that unconsciousness was due to high urea condition called uremic encephalopathy or a bacterial infection because the CT scan done immediately after being admitted showed no change in the clot size and the doctor cleared the fact that there is no neurological reason behind the unconsciousness..  The first hemodialysis was done on 25th  after which the creatinine came down to 3.4,urea at 209,platelet 65000,WBC count 300. Since then till date dialysis is being done on daily basis. After booster dosages WBC count has rised upto 22000 . She was unconscious till 30th of December but now she shows little hand and leg movements, rotates eye balls and responds to sound as well. She is on ventilators since 24th of December .

    Its a kind request that you once review all the facts along with the medications being followed till date and give your suggestions on the case . We would like to know that how can we get her out of ventilators and what is the kidney condition. The cyclophosphamide 4th dosage is due on 10th of January. The present treatment, medications and blood reports are attached within.Your suggestions will be a great help and we will be highly obliged for our this kind act.Waiting for your reply.

    My mom has mpa vasculitis. So it's a little different from Germans but her kidneys really took a hit as well. My mom under went plasmapharesis. Which from my house understanding takes all the bad viruses out of your blood and replaces with good. She also had the cychlophasna(spelled wrong) and high dose of prednisone. She isn't in remission yet but is doing very well. The plasmapheris seemed to really help. Hope everything turnes out ok. Praying
