You actually have a couple of things going on with the head and foreskin area of your penis: you have a relatively extensive skin adhesion between the the foreskin and the head of your penis on the lower-part of of your penis. This itself is not a major issue as this is how my own foreskin has been all of my adult life and it has not caused me any problematic issues.
However, what I think you may have going on is something similar to layering of hard skin that you get on the heal of your feet where the skin of the adhesion meets the skin of the head of your penis.
Currently, where the adhesion is uppermost (nearest to your pee-hole) there are two bumps that I can clearly see. These again are not an issue in itself.
To have an idea what is going on for the skin that I describe as layering (this skin may feel very slightly differently to the skin around it?) I would need to know how frequently you masturbate ...... ?

So what I get is that during sexual function the two parts of the skin that should be separate (the skin of the foreskin and that of the glans / head of penis) split slightly, but not to the level of bleeding or causing other issues.
What I do is give the skin time to heal and perhaps take it more easy on the next occasion.
There are things like you can do: as I say you may need to space the days in which you do and do not masturbate, for example, masturbate one day and take 2 rest days, or more in future should that prove necessary.
You need to understand that your penis will continue to develop naturally until you are around 26 YOA.
The foreskin adhesion may break free itself - and, as long as you are really careful you can do stretches that may assist this process -NEVER try to force the two separate adhered skins apart by using any force. In the main this is something that would happen naturally, but as I say, you can assist the process.
However, it is quite possible that without surgical intervention the skin adhesion may remain - my suggestion would be not to seek a remedy for this now, unless the skin actually splits during masturbation on a regular basis and remedial action has to be put in place.
Your penis and foreskin will develop further anyway, and, the adhesion may break free anyway. If you want further information on the stretches you can do simply hit the Reply text icon
At a future date you may want to get a professional medical opinion on what you can do with the adhesion if it still exists at that time, I would suggest you wait until you are 20 YOA at least, unless you get skin splitting and bleeding issues regularly prior to this.
The specialist to see would be a Urologist, be warned, many simply would say the best approach would be for you to be circumcised - this is completely unnecessary in most cases as a relatively simply incision would more than likely provide a complete resolution to the adhesion.
