There are two typical issues that you may have, or perhaps a combination of things
You may have Balanoposthitis - often the issue is simply that you have a tight foreskin.
have you had issues of having difficulty in exposing your glans / head of your penis before ..... ?

Can you confirm your age? So if you are fully erect you are easily able to expose the head of your penis ..... ?
