what is this condition?

Severe facial pain with buttock pain on the left side of the body. facial and buttock pain variates each day but never goes away completely. concentration, thinking level, perception of 3d environment is weakened and short and long term memory is bad. i cannot do anything or do any activity successfully.

The most observable thing about my condition is my facial tension. i have severe facial tension on the cheek and when i let go of my muscles, my face gets the position seen in the image. It's not like bell's palsy, no eyelid twitching but its like a cheek tension.
