Relax, don't panic. It doesn't seem like anything too serious to me but wait 2 weeks first and then revisit the doctor

thanks im just not sure exactly what it is thats whats worrying me but i will give just thought after 4-5 days i would notice some results

Penile skin, unlike the skin elsewhere on the body, can, take a great deal of time to even begin to heal - this is particularly the case of the skin on the head / glans of the penis. Essentially, healing is dependent on the original cause, and, whether the skin is further exposed to anything that can cause further irritation or damage.
What is the cream you have been given by the Doctor? So, 2 weeks is a very short time for penile skin to notice any particular change, unless there is a simple cause. I would say that, in comparison to fully-healthy adult penile skin your's is quite poor - but that is not unusual, as many men have this level of damage to the skin on their penis.
Without going further into the details of how to fully achieve healthy penile skin it is essential to avoid contact with soap, shower gel and other forms of body wash when you take a shower. Clean the penis with water only - maker sure it is completely dry after washing ....

Had you been sexually active immediately before you noticed this? The econazole is simply an antifungal cream - for many doctors this type of treatment is the first line of defense. In statistical terms this can often sort the problem - it is when the issue is not resolved that you need to be persistent and return to the doctor and get a different type of medication. This may be a steroid ointment for example.
So depending on whether you actually have an infection through sexual contact, such as a typical yeast / fungal or bacterial infection this would need to be resolved.
The other issue your penile skin has that it is generally in quite poor condition, for a doctor seeing that this would not be in any way unusual = once you have sorted a potential infection to your penile skin, you would need to set about improving the quality or healthiness of your, and for most men this means excluding as many chemical based products from contacting your penile skin, of which things like soap and shower gel are top of the list to avoid.
