White burn-like skin. 1 Year balanitis and redness. Treatment does not solve it.

Hello, i have been suffering from white burn-like skin, redness on the head (especially visible on the white skin) foreskin redness, balanitis and random sharp pain that i think is from a spot below the penile corona, near the frenulum (pic attached, note that the corona is red-inflammed and the foreskin has a yellow/brown spot where it touches it). I also had an bacterial infection in my urethra that got cleared with antibiotics from my urologist. (caused random dull pain in the penis head and burning after urination).

I used a few creams that did help but did not solve the issue. The sharp pain went away after i got the spot swabbed and got antibiotics(swab results were bacteria normally living on the skin). The redness did stay and got worse. After that i got prescribed a anti-bacterial cream for regeneration.

All the creams I had always seemed to help for a few days but after that the foreskin goes purple and things go back to redness. The state of it can fluctuate but i never got clear of it.

Two weeks ago i got the sharp pain back again and started using the cream again, the pain is almost completely gone but I have major redness and balanitis, waiting for a doc appointment.

I make sure to clean my penis with only warm water 3-4 times a day, dry it and give it air when i can .. that seems to help a lot. Any soap irritates it and causes it to be super dry, red and balanitis (pic included).

I also noticed my glans can sometimes look decent in the morning, but during the day the redness comes back. Especially on the tip of the penis that is not covered by foreskin. (probably some irritation, so I am wearing loose underwear).

Two weeks(a year ago) before any pain started i had unprotected sex with my partner I am no longer with,

but I had the white skin before.

Any help is appreciated. My docs dismissed the white skin so I still dont know what it is.

I am negative for HIV and syphilis. Waiting for HSV tests (never had HSV like blisters so I hope im clear)

    Could be fungal in origin. Also, washing 3-4 times a day is excessive.

      Also, I think you are maintaining the skin issue with continuation of treatments that aren't working, meaning, the initial presentation did not need intervention. Stop using creams, stop washing it with soaps and just give it a few weeks of water and dab dry and rest from sexual activity to let the body heal it naturally. When all else fails, sometimes we have to step back and see if overtreatment was the issue.

    As you have observed, do not use any form of body wash such as soap on the head of your penis and the entire Foreskin.

    In some instances it is best to wash after each time you urinate - however, the important part is removing any traces of moisture as this can also cause problems

    If you think you have major redness and balanitis go see a Dermatologist

      I am keeping all the moisture away as much as i can.

      I did see two dermatologist a couple of times during the year already. The treatment they give helps but does not solve the issue. (anti fungals and anti-biotics) I was always left with constant redness on the glans and the spot under the frenulum. Right now I am using canesten cream without prescription, that helps a lot with the overall inflammation but the clearly infected red spot is till there and not getting any better.

      I have a derm appointment next week, but i dont think they will help me.

      My HSV tests came back negative.

      I really dont know what to do as the doctors clearly dont fix the issue.

      What most medical professionals would do is to try anti fungals certainly and potentially antibiotics as well - as, in most instances this resolves the issue.

      So when you say:

      this means that you needed to go back or at least update the Dermatologists that were treating you as your situation clearly was not resolved. Once they are aware of this they determine the next most likely source of approach, this again may need amending, as again, what works for other people with similar issues may not necessarily work for you.

      So at the Derm appointment outline what took place and ask clearly: do they know how to resolve what you actually have and what the process is likely to involve.

      Speak of your concerns - ask can they give you the confidence that working together with you they will both be able to resolve the issue in the long-run ......

    i m also having the same issue i have done unprotected sex with a girl ater tht this happens to methn i go to doctr around of 3 month i have this issue my doctor suggst me some test hiv vdrl blood and urine test all test come neagtive but i have some urine infection thn doctor gives me some urine infection medicines n this is fungal my urine infection get cured but fungal not gone thn again my dcotr give me some antifungal cream and a steriod oral medicine antifungul cream does not do as well but oral medicine did a great now my all fungal infection gone there is no patches
