Pretty much any procedure on the prostate has at least some risk of ejaculatory dysfunction post-operatively; that being said, some newer techniques have a lower risk than others. Additionally, surgeons can attempt "ejaculatory duct sparing" procedures, although there is still a risk of damage, as the ductal anatomy can vary among patients.

The male urethra is a long skinny tube. Intersecting with this tube are two smaller tubes; one from each of the lateral lobes of the prostate. These smaller tubes are the ejaculatory ducts. On video cystoscopy (looking into the urethra and bladder with a camera) these intersections appear as two small holes where the urethra and the ejaculatory ducts meet. These intersections are delicate.
The urethra is designed to allow for the elimination of urine and ejaculation. So imagine, if you will, being ablated (having tissue removed) by a rotating blade - which is essentially what a TURP is. Those blades are going to scrape away at the ejaculatory ducts. Nerve function will be preserved - meaning your orgasm will feel the same, but during the post-op healing process those ejaculatory duct openings are going to scar over.
On orgasm, the semen that would normally be eliminated into the urethra is now met with resistance because of the scar tissue, and if the sement can't escape through the urethra, it will take the path of least resistance - meaning that it is harmlessly rerouted into the bladder. A fertility urologist has the wherewithal to reopen those ejaculatory ducts, but the procedure is prohibitively expensive, and it doesn't come with a guarantee. Hope this helps.

That is a given with GL you will have RETRO 9 out of 10 procedure cause it. Sorry that your not fighting for it and just giving up. Hope it works for you and in a month you don't have to have a turp to clean up the mess That has happen to many men on this site I wish you well Ken

Google "ejaculation preserving TURP". No reason same can't be done for GL. You just have to find the right doctor. Also a new procedure on horizon called Aquablation. TURP like results with 75% less chance of retro.

I had GL in December '16. Initially, I had ejaculate one week after surgery. slowly, over the ensuing months, it has diminished to a dribble sometimes and none other times. In my situation, the prostate was growing into the bladder neck; causing my urine flow problems. That made sparing me RE more difficult as described by some of the responses with anatomy dialogue within them. (they were useful and quite good). My urologist said that about 95% of men who choose GL do get RE. So depending on what part of the prostate needs to be vaporized will likely determine whethter or not you will have RE. That should make for good discussion with your urologist.
Many of the prostate medications, Flomax and others, will cause RE while taking it and ejaculate returns once you stop taking it. That might be a good way to see what life is like living with RE. Ultimately, it is not so bad. Yes, I'd rather have the ejaculate, but having little or none doesn't change orgasms or the quality of sex. It is an adjustment; but it is certainly a reasonable trade off for all the misery caused by enlarged prostate. Make more noise when you orgasm so that your partner knows you have "arrived". Best
