I'm Sorry, I don't see anything. What are we looking at???
A description of what you see is always helpful. "Worry" , doesn't tell anyone what your worry is about...and I do have my glasses on and I still don't see anything wrong. In short, it looks perfectly normal.

Im facing this problem over 2 years now.Top of foreskin is swelling up and cracked.Under foreskin is bild up a plaque

Under the foreskin is probably smegma . you need to keep under the foreskin clean . 2 to 3 time a day. Ask the Dr. for Diprolene Ointment. This will help any rash and keep the skin soft so that it doesn't crack.

Okay, what you have is a combination of three seperate conditions here. These are: -

Please answer these questions: What age are you? Have you had problems with your foreskin since you reached puberty (when hair started to grow around your testicles and penis)? How do you normally have your foreskin - always covering the head of your penis, or is it always as you have shown in the photographs?

You may need Medical Professional help to overcome all the issues you have with your penis, so, can you get to a Doctor, or perhaps even a Urologist to help you?

To start with you MUST prevent soap and things like shower gel from getting onto the head of your penis and your foreskin - use water only directly on these skin areas. If you can, expose the entire head of your penis and rinse with the water only when you are taking a shower, you must then dry the head and foreskin area throughly by patting it dry with a clean towel.

As you say 'the top of the foreskin is swelling up' - this is a problem. This is most commonly assoicated with a tight foreskin (Phimosis) and the associated condition of Paraphimosis. What you may also have is Frenulum Breve and that is why the head of your penis is pointing downwards - the natural position for the Meatus (the pee hole) is pointing straight ahead, rather than at the floor.
So, when your foreskin is swollen like it is shown in the photographs can you pull the foreskin forward to cover the head of your penis, or is that too painful to do this?
Probably the best thing for you to do would be to see a Urologist, you can ask your regular Doctor to refer you to one, or ask for a consulatation with a Urologist at a Sexual Health Centre.
The other two problems you have are associated with Penile Hygiene, in the main this has to do with using soap, shower gel and other bodily cleansers to wash your penis with and this can eventually cause the skin to become overly dry, and to crack.
Here is what I suggest you can do about that....

What to do - never use any form of soap on the head of the penis, when you shower, draw the foreskin forward to protect the head of your penis, rinse everything off when you finish showering, then expose the head of your penis and rub with fingers and water only - dry your penis first, carefully pat dry with a towel and make sure the head and foreskin of your penis are completely dry!

For the cracked skin what you need is the right kind of hydration - you should not use regular skin creams, lotions or moisturisers when the skin of your penis is like this. Once you stop using soap to clean your penis you need to start using a natural plant based extract to provide hydration for your skin, for this Aloe Vera Gel or liquid Coconut Oil are the best things to try first. I would try the Coconut Oil - apply this after you have taken a shower or have washed your penis with water only. Apply a light thin coat of the Oil and gently massage this into the skin around the head, foreskin and main shaft of your penis. You may need to do this twice a day to start with, then when your skin starts to recover you can then use the Oil 3 or 4 times a week.
